Durability of heart valve bioprostheses is not satisfactory. This paper begins with the analysis of multiple reasons of BN-2 pericardial valve failure that the durability of the new bovine pericardial valve will be obviously increased by improving valve design and altering fabrication of the valve. The paper also includes the early clinical and hemodynamic evaluation of this new bioprosthesis. 1. Clinical and pathological analysis of BN-2 failureFrom September 1981 to December 1992, 89 patients underwent reoperation in our hospital due to BN-2 failure and the 93 explanted valve were the subject of this analyzed, including 83 mitral and 10 aortic valves. The mean duration of the valves in situ was6.85 years (AV:7.75 years, MV.6.67 years).The % of failure in mitral position (17.5%)was significantly different from that in the aortic (8.2%). Cusp tear with resultant valvular insufficiency was the most common causes of failure. Among the analyzed explants 62.5% ofthe aortic and 72.3% of the mitral valves failed with tears. Calcification was not the main cause of BN-2 failure.Cusp tear occurred mainly in Gabbay type I and II . Many cases appeared absent of grossly appreciable endothelialization ofthe cloth ring on the inflow side of the valve, especially in the stent of post.Enlargement of the original suture holes, from 2 × 3mm~2 to 10 × 10mm2 was found in 6 cases which might develop into Gabbay type I...
