Objective: 1. To discuss the correlation factor of stability of Carotid Artery Atherosclerotic plaque and provide reliable proof for identifying vulnerable plaque of Carotid Artery Atherosclerosis. 2. To observe the clinical effect of stability of Carotid Artery Atherosclerotic plaque with principle of Toxin-resolving and Blood-activating, provide scientific proof for its clinical application in stabling vulnerable plaque. Methods: 1. 120 cases of Carotid Artery Atherosclerosis patients were divided into 2 groups: vulnerable plaque group and stable plaque group compared with 30 cases of normal people. To detect changes of IMT, IMTarea, MIMT, plaque integral, D, IMT/D, stenosis degree, hemodynamics (SPV, EDV, Vm, PI, RI, SR, Blood Flow) of Carotid Artery with high resolving power ultrasound, and the level of hs-CRP,sICAM-1,sVCAM-1,MMP-1,TIMP-1 in blood serum, and observe the relationship between cardiovascular risk factors, indexes of ultrasound, hs-CRP, sICAM-1, sVCAM-1, MMP-1, TIMP-1 and stability of Carotid Artery Atherosclerosis plaque. 2. 150 cases of Carotid Artery Atherosclerosis were divided into 5 groups randomly: Toxin-resolving and Blood-activiting group, Toxin-resolving group, Blood- activiting group, Lovastatin group and control group. The course of treatment was six month. To observe changes of IMT, IMTarea, MIMT, plaque integral, D, IMT/D, stenosis degree, hemodynamics of Carotid Artery with high resolving power ultrasound, blood pressure, blood sugar, blood lipid, hs-CRP, sICAM-1, sVCAM-1, MMP-1 and TIMP-1. Results: 1. There were significant relationship between cardiovascular risk factors, indexes of ultrasound, hs-CRP, sICAM-1, sVCAM-1, MMP-1,TIMP-1 and stability of Carotid Artery Atherosclerosis plaque. 2. The principle of Toxin-resolving and Blood-activating could improve symptoms of Carotid Artery Atherosclerosis patients significantly, decrease IMT, IMTarea, MIMT, plaque integral, increase D, relieve stenosis degree, improve pathologic typing of ultrasound and indexes of hemodynamics. It also could degrade blood pressure, adjust blood lipid, had the effects of anti-inflammation, diminishing degradation of extracellar material. The principle of Toxin-resolving and Blood-activating can stabilize Carotid Artery Atherosclerotic plaque, which was superior to the Toxin-resolving, Blood-activating and Lovastatin. Conclusion: Ultrasound detection of Carotid Artery can be used to clinical monitoring indication of vulnerable plaque. The principle of Toxin-resolving and Blood-activating has the effect of anti- Atherosclerosis and it can stabilize the Carotid Artery Atherosclerotic plaque, which are superior to the principle of Toxin-resolving and Blood-activating. And it is deserved to be spread and applied clinically.
