Objective To analysis of carotid artery plaque conventional signs and three-dimensional power Doppler imaging correlated with cerebral infarction, explore three-dimensional power Doppler imaging technique to detect carotid plaques predict the clinical value of cerebral infarction.Method Conventional carotid artery energy imaging real-time three-dimensional color blood vessels was applied to 42 cases of cerebralin farction group and 50 cases of noncerebral infarction group which were detected carotid plaques by conventional ultrasonic and diagnosed by CT or MRI examination. Using real-time three-dimensional volume probe to observe the size、form、type、echo surface smooth degree、whether or not fibrous cap is complete,and then enter the three-dimensional model to observe the flow situation in plaques, and use the instrument built-in VOCAL software to quantitative analysis for the acquisition of plaques.And using 2? test toexamine the carotid atherosclerotic plaque echoes surface smoothness and the shape.In the end,use Logistic regression to analyze three-dimensional ultrasound sign of carotid plaques and presence of cerebral infarction, Use two independent samples t test to contrast the three-dimensional quantitative numerical between cerebral infarction and non-cerebral infarction group.Result 1. The constituent ratio(59.5%,64.3%)of cerebral infarction group hypoechoic plaque, irregular shape is more than non-CI group(18.0%,38.0%,2? = 16.893,6.309,all P <0.01), the degree of plaque surface unsmoothness of cerebral infarction group is higher than the non-cerebral infarction group(Z =-10.593, all P <0.01), the difference has statistically significant.2. Cerebral infarction group: VI(0.66 ± 0.17), FI(32.56 ± 8.45), VFI(22.44 ± 10.95) compared with the non-cerebral infarction group: VI(0.50 ± 0.18), FI(26.21 ± 6.23), VFI(14.11 ± 8.21).The former group VI, FI, VFI were higher than the average number of the latter group(t = 5.130,5.076,5.035, all P<0.05), the differences have statistically significant.3. The hypoechoic plaque, irregular shape, ulcer plaque, andVI, FI, VFI degrees increased are the risk factors of cerebral infarction, and hypoechoic, irregular echo was moderately associated with cerebral intensity, ulcer plaque and VI, FI, VFI was strongly associated with cerebral intensity.Conclusion 1. The hypoechoic, irregular shape and ulcer plaque in carotid atherosclerosis of routine ultrasonographic features is more likely to happen cerebral infarction2. The three-dimensional power Doppler imaging quantitative analysis of carotid atherosclerotic plaque in VI, FI, VFI, it has more important clinical significance for the and VI, FI, VFI value increase the probability of occurrence of cerebral infarction can.
