| Blood-lipid metabolic disturbance maybe affects the pathogenic factors of acute stroke and the mechanism of acupuncture therapy, which maybe affects the prognosis if the mechanism of acupuncture therapy is involved.There are two equal purposes: 〆xploring the pathogenesis of acute ischemic cerebral apoplexy with hyperlipoidemia (AICAH). (2) exploring the potential modulators to the specific and aspecific therapeutic efficacy to analyse whether lipid metabolism is involved with the prognosis.The research has been devided into two aspects: clinical trail and experimental study. In clinical trial, we demensionally analyzed the pathologic condition of AICAH with logistic regression analysis and distance analysis, meanwhile, we analyzed the efficacy of "Xingfa" by factorial analysis. In the experimental study, we observed the changes of GAP-43 (protein synthesis and mRNA impression), BDNFx NGF, NT-3 and VCAM-1 in specific regions of MCAO rabbits (including the paripheral zone of ischemic area, hippocampal gynus and non-ischemic cortical area) with immunohistochemistry, in situ hybridization method and observed the changes of sVCAM-1 in the serum with ELISA. In the same experiment, the interventing effects of acupuncture on them were studied and the curative effects with different acupuncture therapy methods were compared.The results show that deficiency of liver and kidney with wind stirring inside is the initiating mechanism of sudden onset, phlegm and heat is accompanied with the pathologic process of acute stage. Fire and wind stirring up with disturbance due to phlegm-heat is main inner factor hi the initial syndrome. The production and evolution of noxious heat and blood stasis is due to the entogenous phlegm and the entogenous phlegm is due to pathologic changes of the liver, spleen and kidney. Yangming channel diseases due to phlegm-heat is the main disturbance of channels. Disturbances of channel is the transmission of Zang-fu's pathologic information. Data show phlegm-heat has coherence with the large intestine channel of Hand Yangming. Wind, phlegm, ecchymosis is correlative with gall bladder channel of foot shao-yang. It has not been found that endogenous pathogenic factors have distinct relationship with the production and evolution of other channels' diseases.The analysis of specific therapeutic effect of "Xingfa" show that the therapeutic action is correlative with the parameters of basicranial hemodynamics, VLDL, TT.The changes of these parameters maybe are modulated by the same fators. The improvement of nerve's function is irrelevant to the changes of lipid metabolism (VLDL excluded), blood rheology, coagulation and fibrinolysis. The regulation of "Xingfa" to lipid metabolism may have no effect on mechanism and ways of the efficacy of accupucture therapy, both of which have involved different action. All those suggest that we should search the mechanism of VLDL and TT metabolism as their common regulatory factors and probe the mechanism of acupuncture therapy.Experimental study shows that "Xingfa" can advance synaptic remodeling, advance repair and regeneration of nerve, cut the infiltration of mononuclear-macrophage, inhibit the delayed injure of never cell. The evidence also shows that "Xingfa" can induce the production of GAP-43 and GAP-43 mRNA, induce the impression of NTFs in most cerebral areas, inhibit the secrection of VCAM-1 and sVCAM-1. Among these parameters, advanced GAP-43 protein synthesis is the molecular marker of advancing nerve regeneration and synaptic remodeling.Study also analyses the potential regulatory factors of the aspecific therapeutic action, which gives a new thought and cue for further research. |
