Clinical And Cellular And Molecular Mechanisms Of Acupuncture Treatment Of Simple Obesity

The thesis includes three parts as follows.Literature research: This part had reviewed the etiology and pathology of obesity and collected the acupuncture and herbal medicine treatment for it. In this part, the last clinical and experimental literatures on obesity in the world had been summarized and the literature on acupuncture for simple obesity had been refined theoretically.Clinical research: 89 cases were divided into five groups according to their syndromes which were excess heat in the stomach and intestine, liver qi stasis, spleen deficiency and phlegm block inside, deficiency of both spleen and kidney and deficiency of liver-yin and kidney-yin. All of them were treated by body and ear needle. The result showed that: After the treatment of three courses, the total effective rate was 92.13%. The obesity signs had been improved obviously while the contents of the body weight, sebum fat thickness, BMI, WHR, TG, TC and FBS decreased, HDL-C increased. Compared after treatment to before, it showed the significant difference in statistic. The data indicated that acupuncture had the function of reducing body weight, rectifying blood fat and sugar. The result also showed that Leptin and Ins had decreased which was always high level in blood of simple obesity and the plant nerve balanced index (Y) increased. The data indicated that acupuncture could correct the resistance of Leptin and Ins and regular the plant nerve in simple obesity.Experimental research: Observing the rat's weight and fat signs concluded that fat rats reached 58.9% after being fed on the food of 18% total grease for 16 weeks. The result showed that the feeding prescription was reasonable and the animal model was higher than reported before. Experiment observed the effect in taking food, water, form of fat cell, NPY, Leptin, INS, and NPYmRNA. Conclusion: Acupuncture to fat rat could obviously cut down the taking food and water, reduce the body weight and fat; lessen the quantity, diameter, area andvolume of white fat cell, urge the white fat cell to convert to brown fat cell; effectively decrease the NPY level in hypothalamus and serum. Acupuncture to fat rat could make the level of Leptin and Ins decreased in serum but increased in hypothalamus, correct the resistance of Leptin and Ins. After the treatment of acupuncture, NPYmRNA express was lower than before in rat's hypothalamus. It indicated that regulation to NPYmRNA express might be one of the molecule mechanism of acupuncture to obesity. On the other hand, spontaneous electric frequency and monaminergic neurotransmitter in hypothalamus ARC were observed. Result showed that ARC spontaneous electric frequency of model group was lower than normal group, and enhanced effectively by acupuncture, 5-HT decreased in fat rat's pallium and increased in NTS but they all reversed after acupuncture. It indicated that influence of acupuncture to ARC and monaminergic neurotransmitter might be one of the nerve center mechanism...
