AIM: Severe acute pancreatitis (SAP) regarding as a kind of severe acute abdominal diseases in surgery are often complicated by multiple organ dysfunction syndrome(MODS). Microcirculatory disturbances (MCD) are important early pathophysiological events in various organs during acute pancreatitis. The purpose of this study was to evaluate the role of microcirculation and value of TNF-α /IL-10 in acute necrotizing pancreatitis (ANP) in SD rats; To evaluate the therapeutic effectiveness of Salviae miltiorrhizae on organ's microcirculation and inflammatory mediators in ANP.METHODS: A total of 192 rats were random divided into three group: normal control group 0. 5h, 2h, 6h and 12h (C) ; ANP group without treatment 0. 5hu 2h, 6h and 12h(P);ANP group treated with salviae miltiorrhizae 0.5h, 2h, 6h and 12h (T) . Each group has 16 rats. The ANP model was induced by injection of 5% sodium taurocholate under the pancreatic membrane (4mlkg-1). Normal control group was received isovolumetric injections of 0.9% physiological saline solution using the same method. T group was injected of Salviae miltiorrhizae via portal vein(2g. kg-1). Radioactive biomicrosphere technique (RMT) was used to measure the blood flow at 0. 5h> 2h, 6h and 12h after the induction of ANP. Eight rats in each group was only used for blood flow determinations by the intravenous injections of RMT. The animals were sacrificed at 0. 5h,, 2 h, 6h and 12 h after induction of pancreatitis. The blood samples of the other 8 rats in each group were obtained via superior mesenteric vein to measure levels of BUN, Cr, DAO, TXA2/PGI2. Meanwhile, pancreas, kidney , lung , stomach, distal ileum were collected to observe pathological changes using a validated histology score. Lung tissues were also used for examination of PMN, dry/wet ratio and myeloperoxidase (MPO) activity. Distal ileums were also used for examination of PMN, MPO activity and DAO activity. Kidney was also used for examination of PMN, MPO activity. In other experiment, we also used the same method to investigate the role of TNF-α and IL-10 curing acute necrotizing pancreatitis (ANP) in other rats. The plasma levels of TNF-α and IL-10 were determined by ELISA. Pathological changesin pancreas were studied .The survival period of 15 rats in ANP group and T group were observed.RESULTS: 1. Comparison of ANP group and normal control group: 1. 1 The blood flow in ANP group was significantly lower than that of control group (P <0.05). 1. 2 The changes of pathomorphology: The pathological changes and infiltrating degrees of neutrophils were aggravated significantly in SAP group. The time longer, the severer. The tissue injury was closely correlated with blood flow . Lung W/D ratio in ANP group were significantly higher than those in control group (KO. 01 or KO. 05). MPO activity were significantly higher than those in control group(K0. 01 or KO. 05 ).1.3 pH had no change in ANP group as compared to control group (P>0. 05). 1. 4 Levels of plasma BUN, Cr were significantly higher than those in control group(P<0.01 or P<0.05 ). Levels of plasma DAO were significantly higher than those in control group at 6h and 12 h in ANP group (P<0.01). TXA2 /PGI2 were significantly higher than those in control group at 2h , 6h and 12 h in ANP group (P<0.01). Levels of serum TNF-α were significantly higher than those at 0.5h, 2h, 6h, 12h after establishment of the model (P<0.01 or P<0.05) .Significantly increased the serum concentrations of IL-10 at 6 h and 12h (P <0.0l).2. Comparison of T group and ANP group: 2. 1 The blood flow in T group was significantly higher than that of ANP group at 6h and 12h (P < 0.01). 2. 2 The changes of pathomorphology: The pathological changes and infiltrating degrees of neutrophils were alleviated significantly in T group. Lung W/D ratio in T group had no change than those in ANP group (P >0.05). MPO activity were significantly lower than those in ANP group(P<0.01 or P<0. 05 ). 2. 3 pH were significantly lower than those in ANP group at 2h, 6h and 12h (P<0.01 or P<0.05). 2.4 Levels of plasma BUN, Cr were significantly lower than those in ANP group at 2h, 6h and 12h (P<0.01 or P<0.05). Levels of plasma DAO were significantly lower than those in ANP group at 6h and 12h (P<0.01 or P<0.05). TXA2/PGI2 were significantly lower than those in ANP group at 6h and 12h (P<0.01 or P<0.05). Levels of serum TNF-α were significantly lower than those in ANP group at 2h, 6h and 12h (KO. 01).
