Research On Prevention And Treatment Of TCM Of Chronic Laryngeal Yin With The Theory In Correlation With 'Phlegm And Stasis' And Mode

Study background:Chronic Laryngeal yin is a laryngeal disease mainly associated with chronic hoarse voice, which is similar to diseases such as chronic laryngitis,vocal nodules,vocal polyp, weakening laryngeal muscles,vocal cord paralysis and pre-cancerous larynx condition.The disease requires long-term treatment and the effect of the treatment is unstable.My supervisor,Li Yun-Ying,ever did relevant studies on the disease's related treatment for blood stasis in her pre-clinical studies.It is believed that the disease results from a large number of factors,leading to laryngeal qi stagnation,blood stasis and accumulating phlegm. In the recent years,through continuous clinical practice,many patients suffering from chronic laryngitis,vocal nodules and polyp are apparently influenced by metal elements, emotional obstacles and certain personality characteristics in the early onset.They often occur to a fundamental pathomechanism of qi function irregulation due to qi stagnation and reverse flow of qi.In mental and physical disease theories and clinical studies,enriched experiences with onset mechanism of mind diseases and clinical treatments are accumulated.The common fundamental pathomechanism referring to mind disease is mainly based on qi function irregulation of qi stagnation and reverse flow of qui and results in disease products such as blood stasis,phlegm-rheum,phlegm stasis interblock.Study purpose:1.This thesis focuses on Chinese medical prevention theory of 'disease is treated before its onset.' Its ultimate purpose is to prevent diseases from onset,avoiding and diminishing damage toward human bodies and facilitating human health.2.As modern social environment changes and life and work pressure enhances,the number of patients of chronic laryngitis clinically 'resulting from depression' is increasing.Moreover,a biological medical mode is gradually transferred to one mentality-one social medical mode.Metal and physical medicine draws more and more people's attention.Metal and physical related stand is adopted to doing researches on chronic laryngeal yin,mind disease mechanism and prevention methods,which is taken as bases of clinical practicing theory.3.From the theory aspect,first at all,define the concept of'phlegm stasis relevant' theory, discuss pathomechanism of chronic laryngeal yin,qi stagnation and phlegm stasis interblock and organize document.Study analysisFirst,define the concept of 'phlegm stasis relevant' theory,further discuss mechanism of chronic laryngeal yin,qi stagnation and phlegm stasis interblock and initiate the disease's prevention project.1.epidemical studyChronic laryngeal yin is the most popular disease in laryngeal clinics.The cause of disease is unclear.The public commonly lack preventive awareness and knowledge.There are plenty of treatment methods,all emphasizing sufficient vocal rest and eradicating causes of disease.Therefore,under the guideline of Chinese Medical 'disease is treated before its onset,' the study conducts surveys about dangerous factors and personality behavior characters,which is served as reference for early prevention and protection of the disease's high risky group.1.1 survey of behavior dangerous factors related to chronic laryngitis1.2 survey research of personality behavior character of chronic laryngitis patientsEysenck Personality Questionaire and A Type Behavior Questionaire are adopted to conducting initial survey analysis on a patient's personality and behavior mentality.Metal factors related studies are used to discuss how Chinese Medical mind factors work for chronic laryngitis patients in the onset,which provides basis for how to prescribe medicine to enrich studies of chronic laryngeal yin etiology.1.Chinese medical clinical studyProfessor,Li Yun Ying,has been working on vocal disease's related studies for long. 'Jinhou medicine' is her prescription after years of the clinical practicing and widely adopted to treating chronic laryngeal yin.Based on gi stasis and phlegm coagulation of chronic laryngeal yin,this thesis adjusts Professor's 'Jinhou medicine' and mist it and then inhale it to regulate qi and activate blood circulation,resolve phlegm,nourish vital essence.Observe the treatment effect of 'Jinhou-relevant medicine' toward chronic laryngeal yin.Outcome:1.survey of behavior dangerous factors related to chronic laryngeal yinThe outcome shows females are slightly more than males—its ratio is 1.24 to 1 and 1.57 to 1,widely spreading at diverse age level.The age distribution of two-group patients contains distinctive difference(P＜0.01).The age level of Group B is getting younger.The disease involves different professions and education levels.There is difference of professions and education levels between two groups.In addition,two groups of patients' BMI contain no difference(P＞0.05);drinking cold beverage contains distinctive difference(P＜0.05); smoking,drinking,having curing,fried and spicy foods contain no difference.(P＞0.05) The prevalence time between seasons and everyday life:analysis of two groups indicates the disease take higher percentage annually:40.1%and 32.9%respectively.Among four seasons,there is higher prevalence rate in winter.X~2 exam indicates there is no apparent difference between two groups.According to data analysis,two groups in everyday prevalence time take 60.0%and 68.4%and the consensus of opinion is that no specific prevalence time is found for the disease.Analysis outcome of self-past medical history indicates the three high rates of Group A are chronic digest system disease(14.6%),chronic tonsillitis(13.9%)and chronic rhinitis (12.6%);the three of Group B are chronic rhinitis(14.3%),chronic digest system disease (13.0%)and chronic bronchitis(13.2%).Two groups contain no apparent difference in work areas.(P＞0.05)Daily special cases during everyday life contain difference.(P＜0.05)Two groups contain distinctive difference when there are dust,chemical liquid,extreme temperature difference,working and learning pressure,long time of exposing to air conditioners,poor exercise habits and fewer daily sleeping hours in working environment.(P＜0.01)Furthermore,based on survey of chronic laryngeal yin in China and Taiwan,many other factors are found.The disease contains a variety of differences among different areas but dangerous factors not related to chronic laryngeal yin still cannot be excluded.2.Survey about chronic laryngeal yin patients' personality behavior characterOutcome:two groups' EPQ contains apparent differences in the amount of P,E and N.Group B's P value is higher than that of Group A;Group A's E value is apparently higher than that of Group B.L values between the two contain no difference.According to levels of the disease,different analysis are conducted.Statistics outcome shows TH and CH of the levels of the disease and A type behavior contain clear difference,meaning A type behavior is very likely to influence the development of chronic laryngeal yin.Moreover,the outcome also shows that the levels of the disease and N value of EPN questionnaire contain difference.(P＜0.05)The table of N value is used to test nerve or emotional stability.High scores indicate unstable emotion,including characteristics such as anxiety,depression and strong emotional reaction,pointing out that chronic laryngeal yin patients suffer from certain level of nerve.3.Chinese medical clinic study(1)The outcome of clinical treatment:36 distinctive cases,103 valid cases and 16 invalid cases—total valid rate is 89.7%.(2)Observe improving situation between pre- and post-treatment.The outcome indicates apparent difference.(P＜0.01)It shows medical treatment works out for every stage after medical treatment.
