PurposesThis study was built on the basis of literature research, by using the theory of public goods and public services, from a vehicle-mounted mobile to provide medical services, production, and consumption three angles, summarized the sichuan wenchuan, hunan liuyang jili township hospital and qinghai huzhu county three regions based on on-board mobile medical service pattern. Using the combination of qualitative and quantitative methods, three sample areas of relevant information collection, in combination with diagnosis and treatment based on on-board mobile services, the production and consumption three angles, and access to public health evaluation literature, the evaluation index based on the pattern of vehicle-mounted mobile medical services was constructed. Using the combination of quantitative and qualitative methods to analyze key factors influencing the further development, its sustainable development strategy was discussed..MethodsResearch object:The Sichuan Wenchuan county, Hunan Jili township and Qinghai Huzhu county three areas were determined for the surveyed area. Each region extracted three townships of which three administrative villages were extracted to conduct on-site investigation.Interview object:Wenchuan county mobile medical center, the director of the dean of Liuyang jili township hospital and huzhu county people’s hospital. Main interview content was based on on-board mobile infrastructure situation of diagnosis and treatment, the main service project, management and other related material; The medical service team based on on-board mobile gathered the main configuration and service ability level test and other related material;9township health hospital directores of dean,27village doctores of village clinics. Interview:the understanding based on the vehicle-mounted mobile medical services, function orientation and interagency collaboration situation and related data.Collecting data:The medical services based on on-board mobile was collected,the incidence of three disease outpatient records before and after the two highest annual was carried out, the change of the outpatient expenses except drugs, and MenZhenLiang, outpatient fees and other data was analyzed.Methods; On the basis of literature research and data collection, the qualitative and quantitative methods was combined to investigate the researth. Qualitative research included in-depth individal interviews,nominal group discussion, etc; Quantitative survey includes vehicle-mounted mobile medical service mode based on survey, organization present situation investigation, personnel overall qualities and training situation, etc.Data analysis methods:descriptive research, general statistical analysis.Results1.On the basis of the theory of public services, the circuit generation and development of medical service mode and its role in medical service was analyzed. The three medical service modes of wenchuan.jili and huzhu based on on-board mobile and wenchuan county government supply mode were summarized. In the medical service mode based on on-board mobile, the government undertooken the rural public health services in the capital investment, and arranged the related medical institutions to product, and provided public health services and medical services to consumers, etc. In this mode, the government should provide all the required capital public services; Market supply patterns of jili, medical institutions undertooken the operating funds for vehicle-mounted mobile medical services, according to the production and the market demand to provide public health services to consumers.For-profit,the pattern of using charging methods to compensatoryspending, the market supply mode based on on-board mobile medical services; Huzhu self supply mode incarnated the characteristics of voluntary,which was the embodiment of the social moral level, and to a certain extent, eased the financial pressure.Medical institutions providing public health services in the capital source mainly relied on the government and institutions themselves, made its operation lack solid economic basis, partly through government subsidies, the other partly by fee in order to be self-sufficient.2. Through literature analysis and document research,brainstorming method and the relevant responsible person interviewed by expert, the index of six first-level indicators were determined,respectively,which were the basic configuration, management system, security work, health physical examination work, nursing, health management.The level indicators were decomposed by19secondary indicators.3.Evaluation results showed that:(1) the three sample areas wenchuan scored87.8, jili township hospitals scored70.8,and83.3of Huzhu,what was accord to the reality.(2) The influence factors based on on-board mobile medical service pattern was summarized,which were personnel, assets, management system, service, security work.4. the key problems based on vehicle-mounted mobile medical service mode were:(1) the influence based on factors based on vehicle-mounted mobile medical service provider:1) The function orientation based on on-board mobile medical service was not reasonable;2)The compensation mechanism based on on-board mobile medical services model was not sound;3)The inter-agency collaboration based on on-board mobile medical service pattern was not sound;4) The multicenter governance issues based on on-board mobile medical service pattern.(2) The influence factors of producers based on on-board mobile medical service:1) The human resources factors based on on-board mobile medical services;2)The basic facilities based on on-board mobile medical services were not perfect;3) The follow-up services based on on-board mobile medical services were interrupted.5. the operation environment based on the on-board mobile medical service mode were(1) advantage analysis:1)be the effective solution to the difficulty in seeing doctors and doctor distance;2) provide low-cost services to patients;3) optimize the configuration of medical and health resources;4)Inhibition of the medical expense unreasonable growth;(2) weakness analysis:1)the place unite respond negatively to work about based on the on-board mobile medical service;2) Money hasn’t been invested in based on the on-board mobile medical service by the local government3) Their low income and educational level set constraints on their health consciousness.Discussion and Suggestions1.Bringing the work based on on-board mobile service into the normal medical and health service system to realize routinization management needs to explore.2.The ability of medical institutions based on on-board mobile medical service needs to be strengthened. The job security and salary income based on on-board mobile medical service needs to be improved.3. The medical service based on on-board mobile needs to be linked to the new rural cooperative medical service.4.The cooperation of institutions based on on-board mobile medical service and township and village health institutions needs to be strengthened.Innovation and LimitationInnovation1.At present, the domestic model based on on-board mobile service, of which the system research of the sustainability has not yet been reported, This study first served the vehicle-mounted mobile medical service mode of wenchuan jili township hospitals and huzhu county as a typical case study, which analyzed the factors that affect the sustainable development, and provided valuable practical experience to the promotion of the work based on on-board mobile medical service of our country. 2.Providing theory in the optimization based on the pattern of vehicle-mounted mobile medical services to introduce public goods and public services,in which analyzing the application value of work based on the on-board mobile medical services, under the guidance of the theory optimizing its key areas, improving its model has certain theoretical guiding significance.Limitation1.First on the index filtration, diagnosis and treatment based on on-board mobile service system was relatively large, index of correlation were so many, there was no unified evaluation criterion both at home and abroad, and in the case of indexes selection not enough concise.2.This study is only for mobile medical center, in wenchuan county, jili township hospitals and huzhu county people’s hospital,which has carried on the empirical investigation and study, small sample size, and lack of compared with national mode of similar areas, the strategy put forward based on the pattern of vehicle-mounted mobile medical service and widely applicability in the improvement suggestions remains to be discussed in this paper.
