| Depression incidence increased year by year as the pace of life accel erates, the increasingly fierce competition, the lack of pressure relievin g. Depression is a hard-treated mental disease which happens frequently. And it is caused by a variety of reasons to depressed mood, self experien ce as the center of clinical symptoms or condition. With the rapid technolo gy development of acupuncture, the advantages of acupuncture treatment of depression is attended by all the doctors.The treatment is multi-level, mu lti target, overall adjustment effect point, and it can improve the sleep di sorder with anxiety, depression and other somatic symptoms, no significant s ide effects. Therefore, revealing the characteristics and laws of electro acupuncture in the treatment of depression, using the advantages of acupunc ture treatment of depression and improving the treatment of depression ef f ect has important significance.Objective:To provide a scientific basis and unified standard for electro acupuncture treatment of depression, in this study, the clinical literatures about the electro acupuncture treatment of depression were retrieved, reviewed and analyzed, and summarized the status quo and development tendency of electro acupuncture clinical research in domestic and overseas. We evaluated the quality of clinical research literatures and analyzed principal rule of acupoint selection in electro acupuncture treatment of depression.Research methods:By means of combination with computer and manual retrieval, we retrie ved and reviewed clinical research literatures which published from 1994 to 2014 year in domestic and overseas journals about acupuncture treatmen t of depression. Language of literatures restricted to Chinese and Englis h. The retrieved literatures were systematically reviewedfor relevant stu dies further screening. According to the Cochrane system evaluation handb ook(version 5.0) "simple evaluation method" standard, two independent re searchers adopt "back-to-back" form to assess the independent bias risk o f literature respectively, and based on the involved problems to assess t he quality of each literatures. At the end, the modern literature databas e established to record related acupoint data. Using the Python programmi ng language to analyze literature involved with acupoint In JetBrains Pyc harm Community Edition 3.1.2 software, we summed up the principal rule of meridian selection and point selection in acupuncture treatment of depre ssion. And Treatment parameters, point selection, treatment parameters (fre quency of electro acupuncture, waveform, current intensity and time) analysi s measurement etc..Results:(1) In the overall assessment, there were 2932 (clinical and experimen tal studies) on electroacupuncture treatment of depression literature from 1994 to 2014. The published lit erature number increased year by year, the medium-term (respectively 1994-2000 ye ars,2001-2007 years,2008-2014 year s) literature growth gradient.(2) Among these references, diagnostic criteria mainly for the Chinese mental disorders, classification and diagnosis standard (third edition) "(CCMD-3), a total of 17 articles, earlier literature diagnostic criteria for a small amount of the Chinese classification and diagnostic criteria for mental disorders (second edition)" (CCMD-2), a total of four chapters; Seven, adopt the 10th edition of the international classification of diseases (ICD-10) "mental and behavioral disorder" diagnostic criteria; 3 article USES Chinese mental disease diagnosis standard and classification scheme (CCMD-IFR); Article 2 by the American psychiatric association dsm-iv (MDD) in the diagnosis of depression.(3)In acupuncture treatment of depression, Baihui, Yintang, mainly inN eiguan acupoint. Ex perimental control group of Western medicine mainly use s theSSRIscl ass of an tidepressants f1 uoxetine, of which the most widely used. In the literature of specific acupoints and nonacupoints used of mo re specific, and the ten groups of specific points were used. Specific poin ts overall (404 times) was higher than that of nonspecific points (192); spe cial points in the intersection point (139) of Five Shu Points (106) and the f requency of use was obviously higher than that of other specific acupoint s. Nonspecific acupoints of Baihui Point (to 45 acupuncture prescription, us ing 36)(4)The total of points using frequency was Baihui, frequency of use be fore 10 points respectively:Baihui, Yintang Neiguan, Zusanli, Sanyinjiao, T aichong, Tianting, Sishen cong, He gu. Electroacupuncture acupoints selecte d depression in the prescription, widely distributed in pericardium meridi an, spleen, stomach, liver, heart, intestinemeridians and Du meridian, and Si shencong Yintang for the extra acupoint(5) There were 12 references involved into animal research, and the a nimal models were mainly caused by chronic stress depression which had a certain degree of similarity with that desease of humans. The control gro up mainly used medicine SSRIs antidepressant drug which fluoxetine was the most widely used. The range of the parameters used in clinical acupuncture t reatment of depression is:the frequency of 2Hz, concentrated in the low fr equency, current 1~3mA, strength, mostly to the patient’s tolerance to the s timulus intensity, wave multi use dense wave, continuous wave, frequency of ltimes/d. Electro acupuncture parameter voltage animal re search at 6V,0.5-3 OOMs wide range of wave.11 references of stimulus frequency wasl/d, accoun ted for 91.67%; 1 articles were not mentioned; Retaining time mainly 20 m in, whilce there were no significant correlation between 15 min and 30 mi n the retaining time.(6). There were 33 references involved into clinic research, the use of electric acupuncture treatment of depression of the parameters as follows:1~20 hz frequency, concentrated in the low frequency, a total of 13 articles were mentioned current, the current scope of 1~10 mA; Strength for stimulus intensity is patients tolerance to mostly, waveform choose dilatational wave and continuous wave, more is given priority to, frequency of 1/d; 11 articles were clearly marked voltage to 3 v~10 v, accounted for 33.33%; 6 articles mentioned wave width,0.5~300, Ms; 22 studies and research on HAMD scale as curative effect evaluation index, at 67.67%. Meta analysis, finally into seven articles, by using the random effects model, heterogeneity, statistically significant differences in [SMD= 0.46,92% CI (0.75,0.16)]. According to the electric acupuncture treatment of depression compared with western medicine clinical 5- HT inhibitors of RCT literature meta analysis results showed that the electric acupuncture treatment of depression its curative effect was better than that of 5-HT inhibitors.Conclusion:The treatment of depression by electro acupuncture has exact effect, a nd little side effect. Electro acupuncture is worth the clinical promotio n. Effects of different parameters of electroacupuncture has different trea tment. |
