Analysis And Framework Design Of The Connotation Of The Practical Medical Talents In Township Hospitals

ObjectiveBased on the study of talent related theory, and the investigation and analysis of health service demand in rural areas, this study put forward the concept of practical medical talents, and analyzed the connotation of practical medical talents in township hospitals. In addition, the framework and index system of practical medical talents in township hospitals were constructed, and it provided guide and evidence for the construction and revise of directional talent training project.Methods1. Literatureresearch methodThe theory of talent structure, KAQ, and talent training mode have been reviewed. Then we summarized disease spectrum situation in rural areas, referenced the successful experience of foreign medical talent training, and learned the basic requirements and standards of domestic and foreign medical education.2. Expert consultation methodAfter ensuring the preliminary structure index system of practical medical talents of township hospitals, we used Delphi method to obtain comments and suggestions provided by different experts about index systemof practical medical talent connotation in township hospitals.3. Field investigation methodFour cities were selected from four areas (the eastern, southern, western and northern parts of Henan Province), and nine township hospitals were chosen from each city. The investigation object was mainly the deans of township hospitals and clinical doctors of each department, a total of279people were surveyed. In the trial investigation of index system, we chose1200graduating and college students,as well as300township hospital doctors. At last, the confirmatory survey of index system was conducted.4. Data analysisThe effective questionnaires were classified and settled, and the databasewas established by Epidata software, then the data was analyzed by SPSS15.0statistical tools.Finally, the analytic hierarchy process (AHP) was used to establish the index weight of talent connotation.Results1. Put forward the concept of practical medical talents in township hospital, and analyze related concepts. The practical medical talents, applied talents and GPswere divided according to different standards, they were not contradictory, and they supported and complementedeach other. It provided reference and theoretical support for the medical talenttraining.2. Investigation of the health service demand in rural area. In the investigation of hospitalized disease categories in the township hospitals, there were883common disease records, and the top20common diseases accounted for87.1%of the total frequency. Therewere1199records of common key technologies, and the top20common technologiesaccounted for78.5%of the total frequency. The oral drug, intravenous infusion, operation, acupuncture etc. were the most common treatment technologies.3. Analysis of rural disease spectrum.The rural survey showed that higher hospitalizing rate within two-weeks diseases were acute upper respiratory tract infection, acute gastritis, heart disease, high blood pressure, injury poisoning etc. Those kinds of clinic diseases were more concentrated and easier to diagnosis and treatment.We found that the diseases with higher frequency of hospitalization rate concentrated in respiratory system, digestive system,obstetrics and gynecology etc. The injury and intoxication took a second place.4. Establish and verify the practical talent structure index system. Based on the analysis of practical medical talent connotation and the relationship between knowledge,ability and quality, the index system was divided into three levels, a total of138items, and the validity and reliability of the indicators were investigated. The1260undergraduate students and300 doctors of township hospitals were selectedin verify investigation, and it would be the referenceto follow-up studyby understanding the respective advantages inthose three groups.Conclusionl.The knowledge, ability and quality is the key factor of connotation of practical medical talent.The connotation analysis of practical medical talent is based on theory of talent connotation structure. Consisting of three elements, namely knowledge, ability and quality, KAQ theory clarifies the internal relationship among these three elements in talent structure, which corresponds to the rules of talent training. As a result, three elements is the key factor of connotation formation of practical medical talent.2. Standars of medical educationare the essential basis of defining the connotation of practical medical talent. As the talents educated by higher medical education, practical medical talents’education must meet the basic requirements of medical education. Only the connotation analysis of talents must follow the medical education standards can the quality of practical medical talents be confirmed eligible. Standars of medical education have comprehensive and intensive requirements to talents training, which is the essential basis of medical talents training.3. The connotation definition of practical medical talents must meet three basic functional requirements of township hospitals.Practical medical talents serve for township hospitals, and talents training must be based on basic medical care, public health service and public health management. At the same time, talents training must pay attention to the dislocation and complementary relationship with superior hospitals on duty. Only by this can practical medical talents adapt to the work environment, meet the rural residents’demands and be the practical talents in rural areas.4. The connotation of practical medical talents should take into account both the knowledge’s comprehensiveness and the skill’s applicability.In rural areas, the diseases with high frequency are relatively concentrated, and including common diseases, frequently-occurring diseases and chronic non-infectious diseases, whose diagnose methods are not very difficult and easily grasped with common treatment. In a word, the knowledge which practical medical talents need acquire should be not accurate and specialized, but extensive instead. The professional skill should be applicable, hence practical medical talents need acquire necessary skill of TCM diagnosis and treatment, in order to choose economic and effective diagnosis and treatment methods for the rural residents.5. Practical medical talent connotation need to be transformed to index system, it can be useful for talent training.This studyanalyzed the practical medical talent connotation to providethe practical medical talent cultivationwith guide and evidence, and to make thepractical medical talent cultivation meet the actual requirements. Soonly the connotation of practical medical talents was operable can it serve the cultivation of practicalmedical talents, and the index system of practical medical talentconnotation was confirmed by Delphi and AHP methods, it hadthree levelsand138indicators, and every indicators were assigned weight values to providethe guide and evidence for practical medical talent cultivationin township hospitals.Characteristics and Innovation1. Analyzed the practical medical talentconnotation in township hospitals. This paper summarized the responsibilities and functions of township hospitals, investigated their common key technical means of diagnosis, summarized the disease spectrum of rural areas, studied the training standard of medical education talents, and carried on the analysis to the practical medical talents connotation of township hospitals on the basis of the above research.2. Constructed a practical medical talents framework of township hospitals. Guided with the talent structure theory, oriented by the basic health services, and based onthe analysis of practical medical talent connotation, the index framework of practical medical talent connotation in township hospitals was constructed.The limitation and insufficiency1. Concerning with the investigation objects, this paper selected nearly279doctors in Henan province, which can represent the situation of the Midwestern China, but not that of the Whole country. As a result, the study results of this paper need further research.2. In the investigation of disease categories and key technical means of diagnosis of township hospitals, the disease spectrum is regional in a certain degree, which can’t be on behalf oflarger regional situation.3.The quality of talent training is an important basis for the validation of the index system, but talent training is a long process. So the further optimization of the index system needs a long time, the short term is difficult to achieve good results.
