| ObjectiveThe purpose of this study was to observe the Bloodletting treatment on Shaoyang Meridian points of migraine, and with the traditional acupuncture group as control, to evaluate the efficacy and safety of bloodletting therapy objectively, to explore the mechanism of bloodletting treatment on migraine preliminary, and to provide a basic theory for the clinical application about use of bloodletting to treat migraine.MethodsOur research had collected 60 patients who were diagnosed with migraine and met the inclusion criteria,60 patients all came from Acupuncture Clinics in the First Affiliated Hospital of Guangzhou University of Chinese Medicine and Colleague Chinese Medicine Centre in Hong Kong. The research was a randomized controlled trial,60 patients were divided into two groups averagely according to a simple random numbers method, and there were 30 patients in the bloodletting group, the observe group, and there were 30 patients in the pricking acupuncture group, the control group. The modern medical diagnostic criteria for migraine consult the International Classification of Headache disorders raised by International Headache Society (IHS) in the year 2013, including migraine with or without aura both. The traditional medical diagnostic criteria for migraine consult the syndromes of migraine in the traditional Chinese medicine and clinical research guidelines raised by the country’s administration for Traditional Chinese medicine. And the empirical syndromes of migraine patients were collected, including migraine due to blood stasis, hyperactivity of liver yang, and phlegm syndrome. Methods of treatment:observe group:we select the points jiaosun, zhongzhu, zulinqi as the first set, and sizhukong, waiguan, qiuxu as the second set of points. One set of the points were pricked by needle until there is a little blood flow out, and next time, we pricked the other set of points. Two sets of points were operated alternately in case that there is enough time for the needle wound to be repaired. Control group:we select taiyang, fengchi, shuaigu, xuanlu, touwei on the painful side, and taichong, waiguan, zulinqi on both sides. The points were operated by ordinary acupuncture. All patients in both observe group and control group accepted treatments every two days, there were five times of treatments in one session, and two sessions’were operated in total,20 days were needed altogether. Evaluation standards of efficacy includes:Visual analogue pain scale(VAS), integration of headache scale, Migraine Specific Quality of Life Rating Scale(MSQ). The evaluation standards of efficacy for analgesia immediately depends on the scores the patients in two groups had got, and the scores were evaluated by the time when patients accepted treatments after half an hour, two hours, and six hours. Evaluation of comprehensive efficacy depends on the scores patients had got in the integration of headache scale.ResultsThere were 60 patients took part in the study, and there was no people who did not complete the test.According to the analysis for results by baseline of patients in bloodletting group and patients in needles acupuncture group, and the baseline includes patients’ages, genders, and courses of the disease. And includes scores patients had got in Visual analogue pain scale (VAS), integration of headache scale, Migraine Specific Quality of Life Rating Scale (MSQ) respectively before the treatments in addition. Integration of headache scale includes times of episodes that patients had per month in two groups, patients’ degree of headache when migraine attacked in two groups, the length of time for every attack lasted of patients in two groups, and associated symptoms when migraine attacked. Migraine Specific Quality of Life Rating Scale (MSQ) includes evaluation for limited functionality, disfunction, and emotional impact due to migraine. The results show:There was no significant difference in Indicators above between two groups. (P>0.05). There was comparability between patients in the two groups.Comparison of VAS scores between two groups before and after treatmentThere was no significant difference of VAS scores between two groups before treatment. (P>0.05). There was comparability between patients in the two groups. There was significantly reduce of VAS scores evaluated at the time when it was half an hour after the treatment compared to that before the treatment in both groups, as the same, there was significantly reduce of VAS scores evaluated at the time when it was two hours and six hours after the treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, patients in observe group got lower VAS scores than that in control group when it was evaluated at the end of half an hour and six hours after treatment. There was significant difference by statistical analysis. (P<0.05). There was significantly reduce of VAS scores patients got evaluated after two courses of treatment compared to that before the treatment in both groups. And patients in observe group got much lower VAS scores than that in control group when it was evaluated after two courses of treatment. (P<0.05). The results showed:Evaluation of analgesic efficacy between two groups, both bloodletting therapy and acupuncture needles had a good effect on migraine about immediate pain relief. And bloodletting therapy had a better effect on migraine for immediate pain relief than that of acupuncture needles therapy. Evaluation of pain relief for long-term between two groups, both bloodletting therapy and acupuncture needles had a good effect on migraine about long-term pain relief. And bloodletting therapy had a better effect on migraine for long-term pain relief than that of acupuncture needles therapy.Comparison of Integration of Headache Scale scores between two groups before and after treatmentComparison of episodes between two groups before and after treatmentThere was no significant difference of episodes scores between two groups before treatment. (P>0.05). There was comparability between patients in the two groups. There was significantly reduce of episodes scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was no significant difference of episodes scores between two groups after two courses of treatment. (P>0.05). The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine about reduce of episodes. But the two treatments respected considerable effect to reduce the number of migraine attacks for migraine sufferers per month.Comparison of headache degree between two groups before and after treatmentThere was no significant difference of headache degree scores between two groups before treatment. (P>0.05). There was comparability between patients in the two groups. There was significantly reduce of headache degree scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was no significant difference of headache degree scores between two groups after two courses of treatment. (P>0.05). The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine in relieving headache degree. But the two treatments respected considerable effect to relieve headache degree for migraine sufferers.Comparison of duration of headache between two groups before and after treatmentThere was no significant difference of duration of headache scores between two groups before treatment. (P>0.05). There was comparability between patients in the two groups. There was significantly reduce of duration of headache scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was significant difference with headache duration score of patients between two groups according to statistical analysis. (P<0.05). And patients in observe group got much lower duration of headache scores than that in control group when it was evaluated after two courses of treatment. (P <0.05). The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine in reduce of headache duration. The headache duration of patients in observe group decreased more than that in control group. And bloodletting therapy had a better effect on migraine for headache duration reduced than acupuncture needles therapy.Comparison of associated symptoms between two groups before and after treatmentThere was no significant difference of associated symptoms scores between two groups before treatment. (P>0.05). There was comparability between patients in the two groups. There was significantly reduce of associated symptoms scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was significant difference with associated symptoms score of patients between two groups according to statistical analysis. (P<0.05). And patients in observe group got much lower associated symptoms scores than that in control group when it was evaluated after two courses of treatment. (P <0.05). The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine in relieving associated symptoms caused by migraine. The headache associated symptoms of patients in observe group decreased more than that in control group. And bloodletting therapy had a better effect on migraine for relieving associated symptoms caused by migraine than acupuncture needles therapy.Comparison of MSQ scores between two groups before and after treatmentComparison of limited functionality between two groups before and after treatmentThere was no significant difference of limited functionality scores between two groups before treatment. (D>0.05). There was comparability between patients in the two groups. There was significantly increase of limited functionality scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was significant difference with limited functionality score of patients between two groups according to statistical analysis. (P<0.05). And patients in observe group got much higher limited functionality scores than that in control group when it was evaluated after two courses of treatment. (P<0.05). The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine in lightening limited functionality due to migraine. The limited functionality due to migraine of patients in observed group increased more than that in control group. And bloodletting therapy had a better effect on migraine for lightening limited functionality due to migraine than acupuncture needles therapy.Comparison of disfunction due to migraine between two groups before and after treatmentThere was no significant difference of disfunction scores between two groups before treatment. (P>0.05). There was comparability between patients in the two groups. There was significantly increase of disfunction scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was significant difference with disfunction score of patients between two groups according to statistical analysis. (P<0.05). And patients in observed group got much higher disfunction scores than that in control group when it was evaluated after two courses of treatment. (P<0.05). The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine in lightening disfunction due to migraine. The disfunction scores due to migraine of patients in observed group increased more than that in control group. And bloodletting therapy had a better effect on migraine for lightening disfunction due to migraine than acupuncture needles therapy.Comparison of emotional impact due to migraine between two groups before and after treatmentThere was no significant difference of emotional impact scores between two groups before treatment. (P> 0.05). There was comparability between patients in the two groups. There was significantly increase of emotional impact scores evaluated after two courses of treatment compared to that before the treatment in both groups. (P<0.05). When compared between the two groups, there was no significant difference with emotional impact score of patients between two groups according to statistical analysis. (P>0.05). And patients in both observed group and control group got similar emotional impact scores when it was evaluated after two courses of treatment. The results showed:both bloodletting therapy and acupuncture needles had a good effect on migraine in the reduction of emotional impact due to migraine. And the two treatments respected considerable effect to reduce emotional impact for migraine sufferers.Comparison of comprehensive efficacy between two groups after treatmentComparison of comprehensive efficacy between two groups, there were 30 patients altogether in observed group, among them,5 patients were cured (16.7%),15 patients achieved a significant effect (50%),8 patients achieved a moderate effect (26.7%), and 2 patients achieved no effect (6.7%), the total effective rate was 93.3%. and there were 30 patients altogether in control group, among them,2 patients were cured (6.7%),8 patients achieved a significant effect (26.7%),14 patients achieved a moderate effect (46.7%), and 6 patients achieved no effect (20%), the total effective rate was 80.0%. There was statistically significant difference between two groups according to the Reddit test by statistical analysis. (P<0.05). The results showed: Comprehensive efficacy in observed group was better than that in control group. In other words, bloodletting therapy had better comprehensive efficacy than acupuncture needle in treating migraine. ConclusionAnalysis of comprehensive efficacy in both bloodletting therapy and acupuncture needle, the total effective rate was 93.3% of patients in observed group, and the total effective rate was 80.0% of patients in control group. And bloodletting therapy had better comprehensive efficacy than acupuncture needle in treating migraine. |
