| ObjectiveBased on the systemic clinical and experimental studies,this paper aims to explore the mechanisms of Ai Tong Ping capsule in treating cancerous pain by theoretic and experimental researches according to the theory of Chinese medicine and supervisor’s clinical experiences.The paper also discusses the pathogen and pathogeneses, pathological factors and nature and therapeutic rules of cancerous pain,and analysis the composition and mechanisms of Ai Tong Ping capsule.In experimental studies,the analgesic effect and its type of Ai Tong Ping capsule were observed in rats.Some factors involved analgesic mechanism of Ai Tong Ping capsule,such asβ-endorphin,substance p, NO,expression of c-fos gene,5-HT were also detected.The result may provide a basis for drug development of Ai Tong Ping capsule,and help to improve research on cancerous pain treated by Traditional Chinese herbs.MethodsTheoretical Studies:According to the theory of Chinese medicine and supervisor’s clinical experience for many years,the pathogeneses of cancerous pain by Chinese medicine is discussed in the theoretic part.The chief pathogens of cancerous pain involve six Exopathogens,internal injury by excessive emotions,improper diet and deficiency of Vital Qi.Accumulation of cancerous toxin,mutual stagnation of phlegm and blood stasis and obstruction of meridians are the basic pathogeneses of cancerous pain,among which the accumulation of cancerous toxin is the key point of the pathogeneses,and phlegm and blood stasis are the major pathogenic factors,the both are cause and effect for each other. The deficiency of Vital Qi is the intrinsic factor in the disease.The location of lesion concerns the related Zang-Fu organs and meridians;the feature of the pathogeneses is deficiency in Ben and excess in Biao and the excess in Biao as main aspect.The therapeutic rules of the disease should be removing cancerous toxin and blood stasis, resolving phlegm to dredge the collaterals,and removing cancerous toxin is the crux of the therapy,and resolving phlegm to dredge the collaterals should be applied from the begin to end.Experimental studies: 1.Effect of Ai Tong Ping capsule on stopping pain.the effect of Ai Tong Ping capsule on pain in mice induced by acetic acid and hot-plate was observed.The results show that the capsule is able to reduce the frequency of body torsion in acid-induced model of mouse,and elevate the pain threshold of metatarsal pain in hot-plate model of mouse.This results indicates that the capsule possess the property of stopping pain.2.The type of analgesic effect of Ai Tong Ping capsule.The different effect of Ai Tong Ping capsule on the threshold of metatarsal pain was observed in two model of rat induced by dry yeast and formalin.The results show that Ai Tong Ping capsule can elevate the threshold of pain in both model and the normal rat.It is conclude that and the type of analgesic effect of the capsule may be central.3.The mechanism of analgesic effect of Ai Tong Ping capsule,the effect of Ai Tong Ping capsule onβ-EP and NO in hypothalamus,pituitary and spinal cord in formalin-induced model of rat were observed.The level the SP and c-fos gene expression in spinal cord were also detected in rats.Results1.Ai Tong Ping exert a central analgesic effect by inhibiting the release of the SP in spinal cord,lower the expression of c-fos gene in spine.The results show that the capsule can decrease the number of c-fos immunoreactive nerve and the content of SP.The studies show that decreasing the afferent injury stimulation in spinal dorsal horn neurons,inhibiting the transmission of pain in the spinal dorsal horn may be one of the mechanisms of central analgesic effect of the capsule.2.Ai Tong Ping exerts central analgesic effect by inhibiting the release of NO in central nervous system.The results show that the Ai Tong Ping capsule can obviously decrease the content of NO in hypothalamus,pituitary and lumber spinal cord in formalin-induced model of rat.It is indicated that inhibiting the release of NO in central nervous system,blocking the hyperalgesia caused by nervous injury may be one of the mechanisms of central analgesic effect of the capsule.3.Ai Tong Ping exerts central analgesic effect by increasing the content ofβ-endorphin in central nervous system.The results show that the Ai Tong Ping capsule can obviously increase the content ofβ-endorphin in hypothalamus,hypophysis and lumber spinal cord in formalin-induced model of rat.It is indicated that the increasing the content ofβ-endorphin in central nervous system may be one of the mechanisms of central analgesic effect of the capsule. 4.Ai Tong Ping exerts central analgesic effect by increasing the content of 5-HT and DA in brain,decreasing the content of serum 5-HT.The results show that the Ai Tong Ping capsule can obviously increase the content of 5-HT and DA in brain of formalin-induced model of rat,and lower the serum 5-HT.It is indicated that promoting the synthesis and release of 5-HT and DA in brain of rats,consequently increasing the release theβ-EP,and blocking the excitability transmission of SP,decreasing the release of 5-HT in platelets may be one of the mechanisms of central analgesic effect of the capsule.ConclusionUnder the guidance of the theory of Chinese medicine,the mechanisms of Ai Tong Ping capsule in treating the cancerous pain were studied with molecular biological techniques.The study shows that Ai Tong Ping capsule may treat cancerous pain by affecting the transmission of nociceptive stimulation and pain sensation in central nervous system.The analgesic mechanism of Chinese herbal prescriptions is very complicated. Multiple factors and targets may be involved in biochemical mechanism.In this study,the chief mechanisms of Ai Tong Ping capsule have been elucidated.This study will promote the drug development for cancerous pain by Chinese drugs,deepen the understanding about the pathogeneses of cancerous pain in the theory of Chinese medicine It also provides the new ideas and methods in research on cancerous pain treated with Chinese medicine. |
