| Ⅰ. The road to success.Professor Ji ZhongQiang was among the first batch of college students after the reopening of College Entrance Examination. In his early academic career, he was sponsored by government to study gastrointestinal tumor genesis at Hiroshima University in Japan. He is diligent, eager to learn, thoughtful, and full of courage to practice. Meanwhile, he has a through knowledge of both western and traditional Chinese medicine. (TCM). Within the field of TCM, he is quite familiar with classics as well as famous writings. Professor Ji laid a solid foundation on the theory of TCM and acquired a significant effect on treatment by studying those famous writings and other’s experiences. Within the field of clinical medicine, he is good at both diagnosis and treatment on common diseases, frequently-occurring diseases as well as emergency cases which shows his talent on medical skills. Professor Ji advocates that clinical medicine should be the supplementary for TCM, and we should study TCM by using the advanced scientific method in order to make the theory of TCM more concrete, objective. In this way, the development of TCM could be enhanced and promoted.Ⅱ. The academic origin and clinical experiences of Professor Ji ZhongQiang1. Concept of wholism, especially the wholism between human being and society. Professor Ji emphasizes the concept of wholism. He believes that not only the human body is a whole, both the human body and nature is also a unified system. Professor Ji adhere the holism and theory of that human being is an integral part of nature from Huangdi’s Inner Cannon Classic. The social property of human being and the principle of treatment according to different patient, area and periods of time were placed a high value by him.2. Pay attention to differentiation of disease and syndrome, especially the pathogenesis and differentiation of disease. Professor Ji thinks that differentiation of disease and condition are very important.He attaches importance on differentiations of syndrome and disease, but he believes that the differentiations of syndrome are more important while etiology and pathogenesis are also crucial during diagnosis and treatment. The principle of treatment is based on etiology and prescription originates from the principle of treatment. Whether a treatment is effective largely depends on the understanding of etiology. Professor Ji’s point of view is that a well mastering on etiology is the first step to be succeed in clinical practice.3. Combined traditional Chinese medicine and West medicine together, studying the theory of blood stasis and promoting blood circulation for removing blood stasis. He emphasizes on research and treatment of blood stasis and thrombosis, the value of tongue inspection in the diagnosis of blood stasis syndrome. On the basis of the doctrine from forefathers and previous studies, he combined with modern medicine to study the relationship between blood stasis syndrome and atherosclerosis risk factors, investigate the effects of anti-atherosclerosis effect. Based on the research of Regulating Qi herbs, he found the evidence of Regulating Qi herbs’effects on blood circulation, which formed the academic thought that Qi and blood is crucial in treatment. The theory of Qi and blood circulation as well as the heat-toxin and blood stasis theory were expounded by him.4. Treat Qi and blood at the same time and emphasize regulating Qi to promoting blood circulation. Professor Ji ZhongQiang attaches importance on activation of Qi and blood circulation. Based on the ancient theory of Qi and Blood, the modern research on TCM and researches by himself, Professor Ji believes that the unbalance of Qi and Blood which finally develops the common syndrome of stasis plays a significant role in the development of many diseases. The pathogenesis of blood stasis is the disorder of Qi, including Qi deficiency, Qi clogging, Qi sink and Qi block.5. Professor Ji is flexible in prescription. He praises highly on Treatise on Febrile Diseases, Synopsis of the Golden Chamber by Zhang Zhongjing. He is good at grasping the main syndrome by using principles from Zhang Zhongjing to treat patients masterly. His prescriptions are always concise and efficacy. Professor Ji also refers to the results of modern pharmacology during his differentiation of syndrome and treatment.6. Professor Ji attaches great importance to the role of spleen and stomach. Professor Ji pay attention to regulating spleen disorders, being good at regulating spleen and stomach in treatment of many diseases. He always pays attention to protecting spleen and stomach in his prescription.HI. Examples of Professor Ji’s main clinical experence:1. Chest Pain Disease (Coronary Heart Disease) Professor Ji thinks that heart blood stasis is the core pathogenesis of chest pain (coronary heart disease).He choose Buyanghuanwu Decoction for blood stasis due to Qi deficiency, Xuefuzhuyu Decoction for Qi-stagnancy and blood stasis, Zhishixiebaiguizhi Decoction and Taohongsiwu Decoction for turbid phlegm and blood stasis, New Xuefu zhuyu Decoction for blood stasis with Qi-stagnancy and phlegm. He thinks long-term disease will result in stasis and will intrude into meridians. So one must regulate meridians and break stasis, usually add eupolyphaga, leeches, bombyx batryticatus and centipede and so on.2. Palpitation (Arhythmia):It is important to make sure which is the primary or secondary deficiency and excess, acute and chronic, then treat them respectively. Those deficient syndromes should be treated by tonifying Qi, warming yang, nourishing the blood, nourishing yin.Those excess syndromes should be treated by eliminating heat, dissipating stasis, eliminating phlegm, reducing watery phlegm. He combines electrocardiogram and other modern check-ups to differentiate the syndrome, and he is good at treating palpitation according to syndrome differentiation from regulating mind and heart, liver and spleen, and in the guide of the theoretical system of Chinese medicine, and doing composition of prescription referring to modern pharmacological research results to improve clinical curative effect. He choose Xiaoyao Powder and Zhizichi Decoction for Liver-Qi stagnation and Liver-fire interfered heart, Guipi Decoction or Zhigancao Decoction for deficiency of both heart and spleen, Guizhilonggumuli Decoction combined with Shenfu Decoction for lack of heart Yang, Shengxian Decoction and Taohongsiwu Decoction for qi sank and blood stasis.3. Vertigo:Professor Ji thinks vertigo is related to pathogenic wind, phlegm, water, stasis and heat, and has a close relationship with liver, spleen and kidney. He often use the methods of tonifying kidney and calming liver, and eliminating phlegm and promoting blood stasis as the basic therapy. He use Tianmagouteng Decoction for liver-yang hyperactivity, Banxiabaizhutianma Decoction and Taohongsiwu Decoction for phlegm and blood stasis, Liuweidihuang Pills for kidney essence deficiency syndrome, Tongqiaohuoxue Decoction for syndrome of blood stasis.4. Turbid Blood (Hyperliqidemia):He thinks this disease belongs to the scope of turbid phlegm", "blood stasis", the pathogenesis is deficiency in Ben and excess in Biao. The Ben deficiency refers to the deficiency of spleen, liver and kidney; excess in Biao refers to turbid phlegm and blood stasis. Pathomechanism lies in phlegm and stasis. The therapy is to tonify kidney, eliminate phlegm and promote blood circulation, and often use Tongmaijiangzhi 1 as the basic prescription. The type of phlegm dampness and blood stasis is treated by using Liujunzi Decoction, Qi stagnation and blood stasis with Xiaoyao Powder or New Xuefuzhuyu Decoction, kidney deficiency and blood stasis with Zhibai dihuang Pills for kidney yin deficiency and Jinkuishenqi Pills for kidney yang deficiency5. Heart Failure Diease (Chronic Heart Failure):Professor Ji Zhong Qiang thinks the functional decline of the heart is mostly due to the deficiency of heart Qi, and it will result in the deficiency of heart yang, water dampness too more, inherent resistance due to blood stasis after a long time, and this belongs to deficiency in origin and excess in superficiality. For blood stasis due to deficiency and collapse of Qi,he use Sanshen Decoction and Lingguishugan Decoction. For the kidney deficiency with blood stasis and water stopping, Shengmai Decoction and Zhenwu Decoction will be used.6. Sleeplessness:Professor Ji pays more attention to the function of liver, spleen and kidney as well as abnormal emotion in the development of sleeplessness, he also good at using mind-cure for those patients. Professor Ji can differentiate the syndrome flexibly.He usually use Shaihu plus Longgumuli Decoction or Danzhixiaoyao Powder for Liver Qi stagnation, Guipi Decoction for qi and blood deficiency, Xuefuzhuyu Decoction for blood stasis, Liuweidihuang Pill and Jiaotai Pill for heart and kidney fail to connect, Banxia xiexin Decoction for qi disorder of ascending and descending.7. Weakness (Cancer):Professor Ji thinks weakness caused by disease of cancer is more likely to be the syndrome of the deficiency in Ben and excess in Biao. The Ben are deficiency of yin, yang, qi and blood.The Biao are dampness, phlegm, blood stasis and tumour. He believes that during any treatment period with cancer, integrity protection is extremely important.He insist nourishing qi with herbs which can increase efficacy and reduce toxicity while patient with chemotherapy. He always obeys the rule which is to strengthen the body resistance so as to eliminate pathogenic factors.Ⅳ Clinical research:Effects of New Xuefuzhuyu soft capsule in patients with coronary heart disease and angina pectoris.Objective:To observe the clinical effects, security and the interference with immune-inflammatory reflect of New Xuefuzhuyu soft capsule in patients with coronary heart disease and angina pectoris by a randomized and contrast clinical research method, which can provide evidence for Professor Ji ZhongQiang using the theory of regulating Qi and promoting blood circulation in treating cardiovascular disease.Method:We divided 120 CHD patients into therapy group and contrast group at random, (n=60). All the patients were given routine western medicine; the therapy group was given Xinxuefuzhuyu soft capsule altogether (4 pills each time, and 3 times per day). The patients had been treated for 28 days. We observed the changes of chest pain, to record and calculate the score of angina, stop and reduction rate of nitroglycerin, electrocardiogram results and blood stasis syndrome score. We also tested the level of total cholesterol (CH)1, low densith lipoprotein (LDL), hs-CRP, the level of IFN-r and IL-4, and the ratio of IFN-r and IL-4 of the patients of two groups before and after the treatment, and then carry on safety monitoring.Results:1. The total score of angina was lower than the contrast group (P<0.05).2. Stop and reduction rate of nitroglycerin was higher than the contrast group (P<0.05).3. The changes of electrocardiogram were the same.4.The syndrome of blood stasis were both improved, the changes of the therapy group was more obviously compared with the contrast group (P<0.01).5. After being treated, the levels of LDL-C,TC, hs-CRP, IFN-r of both groups reduced significent (P<0.05 or P<0.01), and the effect of therapy group was superior to the contrast group (P<0.05 or P<0.01).There was a reduction of the ratio of IFN-r and IL-4 in both groups (P>0.05,P<0.01).which was more obvious in the therapy group (P<0.01)Conclusion:New Xuefuzhuyu soft capsule can improve the symptoms of angina pectoris, syndrome of blood stasis, relieving myocardial ischemia, regulating the level of blood lipids, and it is a safe and effective complex prescription of chinese herbs, it can regulate the ratio imbalance of T cell subset on double-direction and multi-target, and so to inhabit the immune-inflammatory reflect, restrain lipid peroxidation, and intervene the development of atherosclerosis. |
