A Study On The Succession Of Academic Thought Of Song Yiting And The Treatment Of Chronic Congestive Heart Failure (Qi Deficiency And Blood Stasis Type) Of Coronary Heart Disease

[OBJECTIVE]As one of the state-level veteran Chinese medicine experts, Professor Song Yiting is an instructor for academic inheritance, who has been working for more than 50 years with rich clinical experiences and effective treatment, especially, in the Cardiovascular and cerebrovascular diseases. This paper aims to summarize and inherit systematically and comprehensively the academic thoughts of Mr. Song and to sum up the clinical experiences, comprehend the academic thought essence, and integrate different elements of the thoughts so that the accumulation of academic ideas and clinical experiences of Mr. Song for decades can be continuously inherited. Meanwhile, his academic ideas can be constantly carried forward through further summaries and extractions in clinical practices, and positive explorations on the development and innovations over spleen and Cardiovascular diseases will be put forward.Chronic heart failure (CHF) was a compound clinical syndrome and the terminal stage of all kinds of heart disease, characterized by high incidence and high mortality. Currently in the treatmeet of chronic heart failure has made great progress, but with the possibility to increase long-term survival in patients of heart failure treatment still exploring, Chinese medicine in the treatment of chronic heart failure has accumulated a wealth of experience, experimental and clinical research to fully proved that Chinese medicine can improve chronic heart failure syndromes, and it also can significantly increase cardiac contractility, improve cardiac hemodynamics, and significantly improve the patients quality of life. Therefore, to carry out research on the prevention and treatment of chronic heart failure in Traditional Chinese Medical research is of great significance.He emphasized that human, heaven and earth should be a trinity and the constitution and the influences of environmental factors should be considered in diagnosis and treatment.[THINKING]Professor Song Yiting got high education in Traditional Chinese Medicine in his youth, and he has got lots of accumulation in TCM study through the extensive study and research. In 1978, Mr. Song got admitted as a postgraduate by the cardiovascular major combining TCM and western medicine, Shanghai College of Traditional Chinese Medicine. He had his knowledge come from Professor Wu Shishu, Professor Qiu Peiran, Professor Huang Wendong, Professor Gao Zhaoji, and master in cardiovascular of western medicine Professor Chen Haozhu. Mr. Song had made researches in the overlapping area of Chinese traditional medicine and western medicine such as cardiovascular diseases, high blood pressure. Professor Song read classics, especially praise highly with <Huang Di Nei Jing>,<Shang Han Lun>, the Prescription he used mostly rooted in the classics, Professor Song Yiting has made complete and in-depth study on Chinese medicine classics and thoughts of other masters. On his treating cardiovascular disease, he has formed his unique theory based on the inheritance of the achievements by predecessors, yet, he has not been fettered by the accepted theories, and he has established academic thoughts of putting emphasis on the holism concept.In recent years, principles of treatment in western medicine changed from cardiac stimulating, dieresis and vascular relaxation to blocking myocardial remodeling factor and stopping and reversing myocardial remodeling, greatly decreasing case fatality rate and mutilation rate of CHF. This disease from the decadency of visceral-Qi, and deficiency of vital energy, blood or body fluid running. It also relates to the accumulation of phlegm, blood stasis、moisture stop poly, which blocked heart vessel, and inhibited the heart-Yang. When we treat this disease, we should note that it is manifested in the deficiency in the five internal organs, excess in superficiality and based on Yang Qi failure in heart.Professor Song thought syndrome mingles CHF’s pathogenesis was a vicious circle of deficiency stagnation with excess caused by deficiency of yang, of blood stasis and fluid-retention stagnation in the interior after he studied a lot of TCM classics. Therefore, we need to think over all of these, warming Yang and benefiting Qi and removing blood stasis and promoting diuresis were the main therapeutic methods. Warming Yang and benefiting Qi and promoting blood circulation for removing blood stasis and promoting diuresis should be used simultaneously.[THE CLINICAL STUDY]Qi deficiency and Blood Stasis of Chronic heart failure treatment and Clinical Study.[PURPOSES]Observation of Yiqiwenyang-qiangxin decoction Qi deficiency and Blood Stasis treatment of chronic heart failure clinical efficacay and safety to explore the treatment of cardiac Yiqiwenyang-qiangxin decoction of chronic heart failure, clinical use as a guide to provide a reliable theoretical basis.[METHOD]Qi deficiency and blood stasis in patients with chronic heart failure randomized controlled by 62 cases of law were randomly divided into two groups, control group (31 cases) the use of conventional western medicine treatment, the treatment group (31 cases) in the control group based on the use of Yiqiwenyang-qiangxin decoction,4weeks of treatment. Observed before and after treatment efficacy in patients withcardiac function, heart failure, TCM Syndrome points, single effect of TCM Syndrome, Six minute walking test.[RESULTS]Yiqiwenyang-qiangxin decoction conventional Western medicine combined therapy can significantly improve the clinical efficacy in patients with heart failure, compared with the control group were different (p<0.05);Yiqiwenyang-qiangxin decoction conventional Western medicine combined therapy can significantly improve cardiac function in patients with heart failure classification, compared with the control group were different (p<0.05);Yiqiwenyang-qiangxin decoction joint conventional Western medicine can significantly improve the scores of Chinese medicine in patients with heart failure syndrome effects, compared with the control group were different (p<0.05);Yiqiwenyang-qiangxin decoction joint conventional Western medicine can significantly improve the treatment of heart palpitations, shortness of breath, chest tightness, cough, expectoration, oliguria, edema, abdominal distension, aversion to cold, fatigue syndrome sweating, TCM individual syndromes compared with the control group have differences (p<0.05),surface floating limb swelling two groups had no difference (p>0.05).Yiqiwenyang-qiangxin decoction joint conventional Western medicine can significantly reduce the chronic heart failure patients with NT-proBNP has the difference (p<0.05).Two groups of patients group were no adverse reactions after treatment on cardiac Yiqiwenyang-qiangxin decoction of toxic side effects on the human body did not, is safe and effective.[CONCLUSION]The treatment of cardiac Yiqiwenyang-qiangxin decoction deficiency and blood stasis in patients with chronic heart failure is effective and can significantly improve the efficacy in patients with heart failure, clinical symptoms and quality of life.[THE INNOVATION]Besides the teacher’s point of view, the writer put forward that dispersing and descending the lung Qi, and regulating the blood to cure the coronary heart disease and chronic heart failure.
