| Professor Liu Ruifen, Shandong Traditional Chinese Medical University doctoral tutor, former director of gynecology, director of gynecology teaching and research center of Affiliated Hospital of Shandong University of Traditional Chinese Medicine. She is the tutor for fifth batch national traditional Chinese medicine experts in the academic experience inheritance, and has inheritance studio for famous old doctor of traditional Chinese medicine. She has been engaged in clinical medicine, research and teaching work for more than 30 years, committed to the prevention and treatment of chronic pelvic inflammatory medicine, and birth control caused abnormal uterine bleeding, infertility research. After years of clinical repeated verification, absorbing, and with the rich experience accumulated, Professor Liu Ruifen has formed her own unique academic style and made remarkable achievements that have greater impact at home and abroad. She is the one, for the first time, who proposed "blood stasis and kidney empty" theory in chronic pelvic inflammatory disease, and the academic viewpoint and method of stasis, heat and empty theory in treatment of abnormal uterine bleeding caused by birth control measures. She created a curative effect of chronic pelvic inflammatory disease TCM comprehensive treatment program. And she advocates to go through a four-step (Bu Tiao Wen Tong) in staging ovulatory dysfunction infertility treatment; and advocates Bu Shen Huo Xue in treatment of gynecological diseases; stresses the importance of the stomach, TCM drugs to nursing care stomach first. This paper introduces the gynecological academic thought and clinical experience of Professor Liu Ruifen, and observed the effects of Zhi Bao Geng An prescription in the treatment of menopausal syndrome to explore the relationship between ET, NO sweating and hot flashes. Paper is divided into three parts:Part I Professor Liu Ruifen’s academic origin overview.Father opened her way of a career in medicine for Professor Liu Ruifen, and the harsh living environment cultivated her perseverance, hard-working and steady quality practical personality. A comprehensive system of college education laid her a solid foundation of combing Chinese medicine. In particular, her study in Western medicine made a foundation for her to combine Chinese and Western medicine method in clinical treatment of gynecological diseases. She widely read academic experience of ancient Chinese medicine masters opened up her ideas; Inspiration by Chinese culture formed her noble ethics, truth-seeking spirit, and the superb skills. At the same time, she advocates that the woman is born with blood and Qi. She created a complement, adjustment, warm, circulation four-step staging ovulatory dysfunction infertility treatment method. She proposed chronic pelvic inflammatory disease "blood stasis and kidney" theory; she created a curative Chronic Pelvic Inflammatory Disease quadruple therapy; she emphasized abnormal uterine bleeding caused by birth control measures in blood stasis and heat tonic for the treatment.Part Ⅱ Research on Professor Liu Ruifen’s academic thought and clinical experience.1. Professor Liu Ruifen keeps the view that a lot of gynecological diseases are related to "Kidney Blood", and identifying diseases and syndromes, macro and micro should be combined. She emphasizes to regulate emotions, and use TCM drugs to care stomach first, and she stressed the need to distinguish between clinical treatment specimens priorities.2. Professor Liu Ruifen, firstly proposed "blood stasis and kidney empty" theory chronic pelvic inflammatory disease, which was included in textbook<Gynecology of integrated Chinese and western medicine> during "Twelfth Five-Year-Plan" for Medical Colleges. Meanwhile, based on the principle of Activating blood circulation to dissipate stasis, Regulating Qi to alleviate pain, and Invigorating the kidney to Pei Yuan, she firstly made the effective prescription that takes both virtual and actual, notification and purgation, to treat the pelvic infection disease, with reasonable process into granules (now widely used in clinical as hospital self-made prescription). Clinical applications have made significant effect, while the particles carry out clinical efficacy of pelvic inflammatory disease, immunological system and epithelial cell biology research, immunology and blood biochemistry; discussed the pelvic particles to fill the domestic blank. This created the exact effect of chronic pelvic inflammatory disease TCM comprehensive treatment program. Professor Liu Ruifen optimized the treatment options for chronic pelvic inflammatory disease for many times. With optimization and diagnosis standard research financed by the "Eleventh Five-Year" National Science and Technology Support Program project ", on Chronic Pelvic Inflammatory Disease quadruple therapy-chronic pelvic optimization of combination therapy of traditional Chinese medicine, in randomized, controlled, large sample, multi-center clinical studies have shown a significant effect, the recurrence rate is low, it can significantly eliminate and relieve abdominal pain, abnormal vaginal discharge, menstrual disorders and local signs. It can significantly improve the quality of life of patients, and is relatively low cost, safe, can significantly reduce the pain and economic burden of patients. It is suitable for widely promotion. This study defined the chronic pelvic comprehensive treatment program based clinical direction, regulated the clinical treatment of the disease, and improved the range of recognition and application of using Chinese medical treatment in treatment of this disease.3. Professor Liu Ruifen keeps the view that phlegm and kidney are key pathogenesis of endometriosis, and treatment of endometriosis adopts cycle sequential therapy. During menstrual cycle, it take the temporary solution of blood circulation and pain relief, and take thel solution of activating blood circulation to remove blood stasis and relieve pain. While during non-menstrual period, it seeks for fundamental treatment dialectically, same in activating blood circulation to remove blood stasis, but adding invigorating spleen to remove phlegm and tonifying kidney.4. Professor Liu Ruifen proposed menopausal syndrome pathogenesis mainly to kidney, and blood stasis is an important factor that runs through. She introduced Yi Yin Zi Kidney and dispersing liver and activating blood circulation method in treating menopausal syndrome, with attention to psychological counseling.5. Professor Liu Ruifen stressed method of removing blood stasis, clearing heat and notifying deficiency for the treatment of bleeding after medical abortion. For after medical abortion bleeding, the blood circulation is key to the treatment, nourishing Qi to treat basis, aimed at preventing its heat and mass change, created a Gong Qing Granule. National Natural Science Foundation of topic, " Research on using Gong Qing Granule to induce human chorionic cells die in medical abortion bleeding" which is hosted by Liu Ruifen", confirmed that the prevention mechanism and its induced villus, decidua cell apoptosis related. This study is of high academic value in investigating the mechanism of Chinese medicine, guiding clinical, and in seeking for significant drug hemorrhage after drug abortion.Part III Clinical study on ZhiBaiGengAn prescription’s application in menopausal syndrome treatment.Objective:To study Professor Liu Ruifen’s experience prescription ZhiBaiGengAn on the clinical efficacy of menopausal syndrome.Method:The clinical observation used the treatment group (taking ZhiBaiGengAn prescription) and control group (taking Kuntai capsules), treatment lasted two months, clinical symptoms was observed before and after the treatment, which include Kupperman score sheet, serum estrogen diol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), endothelin (ET), and nitric oxide (NO).Results:The effective rate was 73.33% in treatment group, the effective rate was 43.33% in the control group, there was a significant difference (P<0.05) between two groups. There is a significant effect on the treatment group and the control group in improving menopausal syndrome-related symptoms. But ZhiBaiGengAn excels in improving reheat sweating on five upset hot, dizziness, tinnitus, Chest heart palpitations.and other symptoms compared with the control group (P<0.01). After treatment, FSH, LH and before treatment significantly lower, the difference was significant (P<0.05); there are significant differences (P<0.05) compared with before treatment groups E2 levels after treatment. Comparison between the two groups after the treatment group E2, FSH had a significant difference (P<0.01), LH was no significant difference (P> 0.05). ET, the difference was significant (P<0.01) between the NO group after treatment; After treatment, ET/NO were lower, there was a significant difference (P<0.05) compared with before treatment, the difference was significant (P<0.05) between the treatment groups.Conclusion:ZhiBaiGengAn prescription has goodtherapeutic effect on menopause syndrome. And the therapeutic mechanism might be like this:to increase/the ovarion hormone secretion level,and adjust the mutual equilibrium of ET and NO, by adjusting the function of hypothalamic-pituitary-ovarion axis. Hence, bring full play of its physiological effect to tissue and organ to cure themenopause syndrome. |
