Objective:To explore the effect and working mechanism of Bushencupailuan decoction for PCOS through observing its effect on PCOS people's follicle diameter, ovulation rate and pregnancy rate and abortion rate, as well as PCOS model rats' ovarian morphology, sex hormone levels, anti-Mullerian hormone (AMH), granular cell aromatase expression and granulosa cell apoptosis.Methods:1. Clinical research:outpatient collection of 53 patients with polycystic ovary syndrome of ovulation function barrier sterility, randomly divided into two groups, the traditional Chinese medicine (TCM) CC group of 27 cases, the CC group of 26 patients, to observe the dominant follicle diameter, total ovulation rate, endometrial thickness, period of pregnancy rate and abortion rate.2. Experimental study:Bushencupailuan decoction was applied on PCOS model rats induced by DHEA.Examine the morphology of ovaries, detect the serum sex hormones and AMH, detect granulosa cells aromatase mRNA and protein expression by RT-PCR and immunohistochemistry, detect granulosa cell apoptosis rate by TUNEL, detect caspase-8, caspase3,FASL,FAS protein expression in granulosa cells by Western blot and immunohistochemistry.Results:Clinical parts:Bushencupailuan decoction can increase diameter of dominant follicle (P<0.05), and improve the total ovulation rate (P<0.05), increase endometrial thickness (P<0.01), improve the morphology of endometrium (P<0.01), improve the period of pregnancy rate (P<0.05), reduce the abortion rate (P<0.05), so as to improve the pregnancy outcome.The experiment part:Compared with the normal group, the ovaries of model group rats had severe diffuse cystic dilatation and cystic cavities of different sizes, each stage in the development of follicle and corpus luteum number were reduced (P<0.01);androgen and AMH are elevated (P<0.01); Granulosa cells aromatase mRNA and protein expression is reduced (P< 0.01), the apoptosis rate is increased(P<0.05), caspas-8, caspase-3, FAS, FASL protein expression is increased,too.(P<0.01).By comparing with the normal group, Bushencupailuan decoction can obviously improve the PCOS model rats' ovarian morphology, increase the number of mature follicle and corpus luteum (P<0.05), reduce sex hormone levels and AMH level (P<0.01), and increase the model rat granulosa cells aromatase mRNA and protein expression (P<0.01), reduce granulosa cell apoptosis rate(P<0.05), lower caspase-8, caspase 3, FAS, FASL protein expression (P<0.05).Conclusion:1. Bushencupailuan decoction can improve the ovarian function of polycystic ovary syndrome of ovulation, increase ovulation rate, increase the endometrial thickness, thus improve the pregnancy rate, reduce the abortion rate, improve the pregnancy outcome.2. PCOS model rats induced by DHEA is close to human polycystic ovary syndrome. 3. Bushencupailuan decoction can reduce the serum androgen and AMH of the PCOS model rats.4. Bushencupailuan decoction can increase the PCOS model rat ovarian granulosa cells aromatase mRNA and protein expression.5.Bushencupailuan decoction can decrease the follicular granulosa cell apoptosis rate and reduce granulosa cell apoptosis rate, lower caspase-8, caspase 3, FAS, FASL protein expression.
