Research And Clinical Observation Of Minimally Invasive Needle Endoscope Therapy On Acute Gout Arthritis About Chemokines And Clinical Curative Effect

Acute gout arthritis(AGA)is one of the common clinical arthritis.AGA is a disease caused by the disorder of purine metabolism and uric acid salt deposition in the joint,which leads to acute pain and swelling of joints.In Traditional Chinese medicine,that occurred due to patients drinking wine and eating fat and so on products for external.The internal reason is that long phlegmendogenous can damage the spleen and stomach,which leads to spleen and stomach deficiency,and also leads to blood deficiency.Minimally invasive needle endoscope therapy is a minimally invasive diagnosis and treatment technology base on Western anatomy andsurgical operation and under the guidance of the theory of traditional Chinese medicine.ObjectiveTo evaluate the short-term and long-term curative effect of minimally invasive needle endoscope therapy of acute gouty arthritis and investigate the mechanism of it.In order to provide a basis theory for clinical application of minimally invasive needle endoscope therapy.MethodsAccording to the criteria of diagnostic and exclusion,42 cases of acute gout arthritis were collected According to the principle of controlled and non blind clinical trial,they had been divided into 2 groups.The treatment group has 21 cases which were immediately given Etoricoxib 120mg per times,1 times per day,and served 8 days,then changed to 60mg per times,1 times per day.On the second afternoon,the treatment group was for minimally invasive needle endoscope therapy.In the control group has 21 cases,after they entering the group were given Etoricoxib 120mg per times,1 times per day,and served 8 days,then changed to 60mg per times,1 times per day.After entering the group,the basic treatment were both given to two groups.And the routine control of uric acid treatment began after two weeks.Then all the patients were followed by six months of observation.The numbers of patients in two groups with attack of acute gout arthritis was calculated for six months.ResultsMinimally invasive needle endoscope therapy has definite effect onthe treatment of acute gouty arthritis,compare to the control group with drugs,it can reduce the level of ESR,C reactive protein,D dimer,IL-1β,IL-6,TNF-α,IL-1β mRNA,IL-6 mRNA and TNF-a mRNA in serum.The difference was statistically significant(P<0.05).It can also relieve the symptoms of joint swelling,pain,tenderness and joint burning,the difference of the joint pain scores has statistical significance(P<0.05).during the six months follow-up,the recurrence rate was lower,the difference was statistically significant(P<0.05).No case was dropped during the observation.During the treatment time,both the treatment group and the control group had no adverse reactions.ConclusionThe short-term and long-term curative effect of minimally invasive needle endoscope therapy in acute gouty arthritis is more significant.And compared with the simple drug treatment group,the minimally invasive needle endoscope therapy has better efficacy,less trauma,short postoperative recovery time,and has no obvious side effect.So it has high degree of acceptance in patients.It is worthy of promotion in clinical application.
