| ObjectiveIn this study, the efficacy of fire needle treatment on acute exacerbation of gout arthritis is under observation。To evaluate the optimization plan of curative effect objectively and provide the reliable evidence for the clinical treatment programs of fire needle treatment on local acupation selection plus standardized western medicine treatment.MethodPatients from department of acupuncture and rheumatology ward of the First Affiliated Hospital of Guangzhou University of TCM were selected from June 2015 to February 201. In accordance with the study we included patients suffered with the standard of gout arthritis with acute episode.60 cases in total, according to the ratio of 1:1, randomly assigned to experimental group (basic treatment+fire needle prick+standard western medicine method) and control group (basic treatment+western medicine method), each with 30 cases. Experimental group in foundation treatment under the premise, using fire needle prick combined with western medicine treatment.①The basic treatment; ②Conventional acupuncture with a fire needle acupoint selection:Dialectical acupoints (for the syndrome of rheumatic stagnated heat, select the ZuSanLi, YinLingQuan, Quchi, NeiTing) combined with YinBai, JieXi,TaiXi and three Ashi points, the above points are ipsilateral acupoints;③The stantard western medicine treatment:the control group in the treatment of using only conventional western medicine therapy (etoricoxib:120 mg,qd; sodium bicarbonate tablets:1 g, tid). Both therapies were applied five times a week. One week was set as one therapeutic course and two courses of continuous therapy were needed for both groups. Patients were evaluated before and after 2nd course of treatment by the Chinese symptoms grading, as well as the VAS rating scale. The SPSS20.0 software was used for statistical analysis.ResultsBefore treatment, two groups of gender, age, course of disease, disease of the general information, the difference was not statistically significant (P>0.05), the two groups has comparability. Meanwhile, two groups of TCM symptoms and signs score and VAS score, serum uric acid, erythrocyte sedimentation rate are at baseline,the difference was not statistically significant (P>0.05), and between groups with comparable. After two courses of treatment, the overall effect of the two groups, the total efficiency of the experimental group was 93.33%, control group, was 100%, the difference was not statistically significant (P>0.05); the analgesic effect, both of groups was 100%, the difference between the two groups has statistical significance (P<0.05). Compared with two groups of Chinese symptoms and signs of total score, joint VAS pain scores were improved (P<0.05). In addition with two aspects of activity limitation, secondary symptoms, which had no difference (P>0.05), the rest of the Chinese symptoms overall improve effect in the experimental group was better than control group(P<0.05).Conclusion1. Under the premise of the same basic treatment (non drug), both of the Fire needle prick combined with western medicine therapy and the Simple western medicine treatment of acute attacking of gout arthritis were effective.2. At the end of the treatment, besides two aspects of Chinese symptom s (limited joint activity and secondary symptoms), about the pain, tenderne-ss, swelling and overall improvement of the main symptoms, the test group was superior to the control group (P<0.05).3. After the course of treatment, the improvement effect of the experi-mental group was better than that of the control group.In view of the acute attack of gouty arthritis treatment, fire needle can effectively relieve pain and improve joint pain, tenderness, swelling symptoms. As a traditional treatment without obvious side effects, fire nee-dle therapy is worth of clinical application. |
