| Objective: To observe the effect of “Qi lian fu zheng capsule” combined with intraperitoneal chemotherapy on preventing recurrence and metastasis after radical operation of colorectal cancer and inhibiting the orthotopic transplanted tumor of mice H22 hepatoma cell and to study the mechanism of anti-tumor action.Methods:(1)Animal experiment: The mice bearing orthotopic transplanted tumor of H22 hepatoma cell were randomly divided into four groups of model group, gavage group, intraperitoneal chemotherapy group and comprehensive group and 10 normal mice were defined as the normal group. The life state, the change of weight and the growth of tumor were observed. The pathologic change was observed by HE stain. The expression of Cox-2, HIF-1α, VEGF-A m RNA was assayed by real-time PCR and the expression of protein was detected by Western-blot.(2)Clinical observation: The patients of coloretal cancer after radical operation were selected as the objects. The cases were divided into control group(venous chemotherapy), observation group 1(intraperitoneal chemotherapy combined with venous chemotherapy) and observation group 2(Qi lian fu zheng capsule combined with intraperitoneal chemotherapy and venous chemotherapy). After 6 cycles of therapy, the changes of clinical symptoms, Karnofsky score, body weight and safety were compared for the three groups and the local recurrence and liver metastasis rates were observed in 2 year of follow-up.Results:(1)Animal experiment: In the aspects of life state and change of weight, the effect of gavage group was better than comprehensive group group(P<0.05) and comprehensive group group was better than intraperitoneal chemotherapy(P<0.05). But in the aspects of growth of tumor and pathologic change, the effect of intraperitoneal chemotherapy group was better than comprehensive group(P<0.05) and comprehensive group was better than gavage group(P<0.05). The expression of Cox-2, HIF-1α, VEGF-A m RNA and protein was obviously up-regulated in the model group compared with the normal group(P<0.01) and was down-regulated in treatment groups compared with the model group(P<0.05). The expression of m RNA and protein from low to high was comprehensive group, intraperitoneal chemotherapy group and gavage group in order(P<0.05).(2)Clinical observation: In the aspects of changes of clinical symptoms, Karnofsky score, body weight, observation group 2 was both better than the control group and observation group 1(P<0.05), but there was no significant difference between the control group and observation group 1(P>0.05). In the aspect of reducing the rate of recurrence and metastasis, the observation group 1 and the observation group 2 were both better than the control group(P<0.05), but there was no significant difference between the observation group 1 and the observation group 2(P>0.05). The gastrointestinal responses of observation group 2 were less than the observation group 1 and the control group(P<0.05) and there were no significant difference in other adverse reactions among groups(P>0.05).Conclusions: “Qi lian fu zheng capsule” combined with intraperitoneal chemotherapy could inhibit the growth of orthotopic transplanted tumor of mice H22 hepatoma cell and its mechanism of anti-tumor action might be related to down-regulate the expression of Cox-2, HIF-1α, VEGF-A m RNA and protein. “Qi lian fu zheng capsule” has some advantages in improving clinical symptoms, Karnofsky score, body weight, and intraperitoneal chemotherapy has the advantage in reducing the rate of recurrence and metastasis. The combination of the two could complement each other and improve the therapeutic effect. In summary, “Qi lian fu zheng capsule” combined with intraperitoneal chemotherapy, as one therapy combined traditional Chinese medicine and western medicine, is effective, safe, and worth promoting. |
