The Study Of Clinical Effect Observition Of Jianpi Xiaodu Drink Based On The Hypothesis Of "spleen Deficiency And Toxic Damage To Stomach Collaterals"on Precancerous Lesions Of CAG And Its Effect On Expression Of Bcl-2,Bax

Objective:Observation Jianpi Xiaodu drink the clinical efficacy of treatment of the CAG precancerous lesions and carry out experimental research,observe the influence of Jianpi Xiaodu drink to the CAG precancerous lesion and gastric mucosa tissues of the Bax,Bcl-2 and its m RNA expression;Discusses the Jianpi Xiaodu drink treatment mechanism of action of the CAG precancerous lesions,deepen and perfect the theory of“Spleen deficiency toxic damage to stomach collaterals”.Method:38 patients with precancerous lesions of CAG who met the criteria of case selection were treated with Jianpi Xiaodu drink for 3 courses(6 days a week and 1 month as a course of treatment),with self-control study.To observe the clinical effect,clinical symptoms,gastroscope grade,histopathological changes of Hp and tongue vein in patients with precancerous lesions of CAG before and after treated.Experimental study,using immunohistochemical detection before and after treatment in patients with gastric mucosal Bcl-2,Bax expression;Using RT-PCR technology,testing the Bcl-2,Bax m RNA expression in the gastric mucosa of treated patients.Results:Clinical research: 1.Comprehensive effect: the total effective rate of 38 patients with CAG precancerous lesion was 84.21;2.Comparison of TCM symptom score: after treatment,the total score of TCM symptom and the score of each single symptom were all decreased,the difference was statistically significant(P < 0.05);3.Grade of gastroscope: after treatment,the grade of gastroscope was improved after treatment(P < 0.01).4.The pathological grade and integral comparison: after treatment,the gastric mucosal tissue atrophy,intestinal metaplasia,dysplasia and histopathology grade decreased,and the score decreased.The difference was statistically significant(P < 0.05).5.There were 25 cases of Hp positive before treatment,the positive rate was 65.8%,10 cases of HP infection turned negative after treatment,the positive rate decreased to 39.5% and the eradication rate of Hp was 40%.The difference was statistically significant(P < 0.05).Tongue vein: tongue coating,tongue quality,tongue body and vein image were all improved(P < 0.05).7.In the process of the drug,the patient has no adverse reaction,treatment after review of routine blood,liver,kidney function,electrocardiogram(ecg),no obvious abnormalities.Experimental research:1.Immunohistochemical method was used to detect the effect of Jianpi Xiaodu drink on the expression of Bax,Bcl-2 in gastric mucosa: after treatment,the positive expression rate of Bcl-2 in gastric mucosa was lower than that before treatment(P < 0.05)and the positive expression rate of Bax in gastric mucosa was higher than that before treatment(P < 0.05),and the immunohistochemical score was lower than that before treatment(P < 0.05).2.RT-PCR method was used to detect the effect of Jianpi Xiaodu drink on the expression of Bcl-2、Bax m RNA in gastric mucosa: after 3 months of treatmentin by Jianpi Xiaodu drink,the expression rate of Bcl-2 m RNA was lower than that before treatment,and the expression rate of Bax m RNA was increased.The statistical analysis showed that the difference was statistically significant(P < 0.01).3.Western blot method was used to detect the effect of Jianpi Xiaodu drink on the expression of Bcl-2、Bax: The relative gray value of Bcl-2 in the treated patients was lower than that before the treatment,and the expression of Bax in the gastric mucosa was obviously decreased.The relative gray value of Bax in the gastric mucosa was higher than that before the treatment,and the ratio of Bax / Bcl-2 was increased.The difference was statistically significant(P < 0.01).Conclusion:1.Jianpi Xiaodu drink has good clinical effect on precancerous lesion of CAG,It can significantly improve the clinical symptoms of CAG precancerous lesion,such as stomach pain,fullness of stomach,eating less appetite,and improving the quality of life of the patients.2.Jianpi Xiaodu drink can improve the pathological grade of CAG patients with precancerous lesion,and improve the histopathological changes of gastric mucosa atrophy,intestinal metaplasia,atypical hyperplasia.3.Jianpi Xiaodu drink can intervene in the progression of precancerous lesions of gastric cancer,and have certain reversing effect on the further development of gastric precancerous lesions,thus preventing the occurrence of gastric cancer.4.No obvious adverse reactions were found in the course of treatment of Jianpi Xiaodu drink,which indicated that the prescription had good safety.5.Jianpi Xiaodu drink can down-regulate the expression of apoptosis-related gene Bcl-2 and up-regulate the expression of Bax in gastric mucosa of patients with precancerous lesions of CAG,and influence the ratio of Bax to Bcl-2 to promote apoptosis,which may be one of the mechanisms of its intervention in CAG precancerous lesions.
