Evaluation Of Therapeutic Efficiency Of Zhenwu Decoction And Xiaoqinglong Decoction In The Treatment Of IBS-D Yang-deficiency And Cold-rheum Syndrome

Aim:To clarify the existence of a new pattern of diarrhea-predominant irritable bowel syndrome,yang deficiency and cold rheum pattern;to verify the curative effect of the compound prescription of Zhenwu Decoction and Xiaoqinglong Decoction;and to explore the formula explanation,the principles of efficacy,the formula pattern,and the correct usage of this compound formula.Methods:During the first two years,48 patients were selected for clinical evaluation according to the diagnostic criteria of integrated Traditional Chinese Medicine and Western medicine.The results of the special symptom score analysis form filled out by each patient before and after treatment were analyzed in detail.The curative effect was evaluated according to the changes of the main symptoms and the total symptoms before and after treatment.Results:In 29.17%of cases a ’clinical cure’ was achieved,in 68.75%of cases a ’marked effect’ was achieved,and in 2.08%of cases the treatment was deemed ’effective’;the total efficacy rate was 100%.This compound formula had obvious curative effect on both the chronic persistent type and the sudden onset type of this disease pattern.The curative effect was rapid:the fastest effect was achieved in 4 days,the majority achieved an effect within 7 days,and the best effect was achieved in 2-4 weeks;the recurrence rate was low,the mind and body were regulated simultaneously,and multiple diseases were affected simultaneously.No obvious adverse reactions or side effects were discovered during the treatment.Symptoms and signs of this disease:The main symptoms are diarrhea and abdominal pain.Secondary symptoms include loss of appetite,fatigue,and lethargy;cold body and limbs,and weak low back and knees;chills,runny nose,and sneezing;enlarged and pale tongue body with tooth marks,and a white tongue coating;and a soggy-moderate or deep-fine-weak pulse.Pattern determination:Two main symptoms plus two or more secondary symptoms.Formula pattern of this compound formula:The complexion is yellow,pale or grey,dull,rarely red or bright,and there may be facial edema;cough and wheezing,nasal noises,excess saliva or sputum and nasal discharge of a thin consistency,lack of thirst,chills,or lack of sweating;dizziness,palpitations,limb tremors,instability while walking,or even inability to stand;inhibited urination,edema,cold and heavy or painful limbs,abdominal pain,or diarrhea;body heaviness and fatigue,listlessness,or slow reactions;a pale,enlarged tongue with a white and slippery coating,or a soggy-moderate or deep-fine pulse.Pattern determination:In the above six groups of symptoms,only one or two symptoms from each group must be present;the presence of all symptoms is not necessary.Conclusion:The existence of the yang deficiency and cold rheum pattern of diarrhea-predominant irritable bowel syndrome was clarified;the compound formula of Zhenwu Decoction and Xiaoqinglong Decoction had a precise and positive effect on the pattern;the compound formula was also effective for other disease patterns of the body caused by yang deficiency and cold rheum;and further taking the compound formula also had the effect of improving the temperament.The formula explanation of this compound formula was discussed;it was confirmed that there exist many formulas within it;the efficacy of the compound formula for warming yang,raising yang,resolving the exterior,dispelling wind,disinhibiting water,resolving spasms,and astringing the intestines was analysed;the correspondence between this compound formula and this disease pattern,as well as the correct use of this fonnula and syndrome,were established.This compound formula’s abilities,via its effect on water qi,to harmonise multiple meridians,multiple viscera,and both interior and exterior simultaneously;to alleviate different diseases with the same treatment;and to affect the mind and body simultaneously were described.This study provides some research ideas and practical experience for expanding the use of compound formulas to treat difficult and complicated diseases in Traditional Chinese Medicine.
