important organ of facial features,which is an important part of the beauty of the appearance.With the improvement of the aesthetic level,people are paying more and more attention to the exquisite repair of the nasal subunit structure,such as the repair of nasal soft-tissue triangle deformities.The main clinical manifestations of which include retraction and notching,and(or)bilateral asymmetry and so on.It is due to cartilage retraction and weak support force.However,how much scope does cartilage recede?What is the distance from the outer edge of the cartilage to the skin margin of the nasal limb?How to choose the right cartilage graft for correcting cartilage retraction and notching?The treatment and nursing work is an important link to ensure the success of the surgery after the nasal plastic surgery.Nose swelling,pain and so on is common symptom after rhinoplasty.The efficacy study of Scutellaria ointment for reducing the swelling and pain after rhinoplasty has important clinical significanceObjective The morphological data of normal human nose was collected by three-dimensional scanning technology.The characteristics of nasal cartilage and nasal soft triangle were understood through necropsy.The results were used to guide the repair of nasal soft-tissue triangle deformities,and study the effect of swelling and pain recovery of the nose as scutellaria ointment was used postoperative of rhinoplasty.Methods(1)Student volunteers,staff,and hospital medical personnel who were enrolled in this trial,120 males and females respectively,collected nasal morphological datas such as nasal length,nose depth,nasal width,nasal wing convexity and nasal soft-tissue triangle withdrawal situation through three-dimensional scanning technique.(2)Take 8 adult cadavers,4 females and 4 males,conduct anatomical studies of nasal cartilage and nasal soft-tissue triangles.Measure the size and thickness of alar cartilage,lateral nasal cartilage,and nasal septal cartilage respectively.Department of pathology was taken to observe the anatomical features of nasal soft triangle.(3)Summarize above basic research results,guide the clinical practice operation of nasal soft triangulation.From January 2015 to July 2016,there were 23 patients underwent surgery.Autologous cartilage was used as transplanted material.During the operation,autologous cartilage of appropriate size was designed and carve in accordance with the retraction and collapse of the alar cartilage.In order to reconstruct the nasal alar fornix and correct the nasal soft triangle deformities.By measuring the changes of the angles of the inner and outer alar cartilages,the withdrawal of the nasal soft-tissue triangle,and the improvement of the nasal contours by comparing the pre-and postoperative images,the satisfaction of the operation was investigated to evaluate the surgical results.(4)Selecting 45 patients undergoing rhinoplasty from January 2016 to July 2017 were randomized into a test group,a test control group,and a blank control group,each group had 15 cases.The test group was treated with scutellaria ointment,the test control group was treated with chlortetracycline eye ointment and the blank control group was not coated with any drug.After regular follow-up visits,the swelling of the nose was generally observed,the volume of the nasal part was measured with a three-dimensional camera,the pain index was measured,and postoperative satisfaction survey,in order to evaluate the efficacy of scutellaria ointment in promoting the regression of nasal swelling and pain relieving.Results(1)The nasal morphological data of normal adults can be measured by three-dimensional scanning technology:the male nasal length is 42.421.10mm,the nasal base width is 33.67±0.99mm,the nasal width is 37.23±1.52mm,and the nasal depth is 24.14±0.87mm,right nasal wing convexity value is 19,67±0.69mm,right nasal soft-tissue triangle retreat value is 5.23±0.35mm;female nasal length 39.26±1.30mm,nasal base width 30.57±0.87mm,nasal width 33.93±1.41mm,nasal depth 22.25±0.94mm,right nose nose convexity value is 18.86±0.69mm,right nasal soft-tissue triangle retreat value is 5.49±0.53mm.(2)Through the study of adult autopsy,the size and thickness of nasal cartilage,alar cartilage,and septal cartilage can be obtained.The distance from the outer edge of nasal alar cartilages fornix to the alar of skin can be accurately measured by an electron microscope,and the value of right is 6.25±0.79mm,the left is 6.34±0.65mm.(3)The clinical practice of 23 cases of nasal soft-tissue triangle correction surgery,effect postoperative is good,the appearance and touch feel nature,no serious complications,follow-up 6 to 18 months,the angle between the inner and outer alar cartilage from the preoperative(42.2 15.2)°,to(64.5±10.2)°after reconstructed,the difference was statistically significant(P<0.05).The nose convexity value was increased after surgery,and the nasal soft triangle retraction decreased,compared with preoperative,the difference was statistically significant(P<0.05),and the postoperative follow-up 6 months,efficacy satisfaction was 91.3%.(4)Scutellaria ointment was used for nasal swelling treatment after rhinoplasty.It can be seen that the nasal volume decrease in the experimental group was significantly higher than that in the control group and the blank control group,with a significant difference(P<0.05).The pain intensity of the experimental group was lighter than that of the control group on the 3rd and 7th day after treatment(P<0.05),but the pain recovery was better on the 14th day,when has no obvious pain in the 3 groups(P>0.05).The experimental group had higher satisfaction than the control group on the 14th and 30th day after treatment,and the difference was statistically significant(P<0.05),but there was no significant difference in efficacy satisfaction between the 7th,90d,and 180d groups(P>0.05).Conclusion(1)The nose shape measurement and anatomy study are conducive to understanding the structural features of nasal soft triangle,and have important guiding significance for the preoperative design and intraoperative operation of nasal soft-tissue triangle deformity collection.(2)Autologous cartilage transplantation to repair nasal soft-tissue triangle deformity,postoperative nasal shape is good,patients with high degree of satisfaction,is an ideal surgical method.(3)Scutellaria ointment can effectively promote nasal anti-inflammatory and relieve swelling after rhinoplasty,speed up surgical recovery,improve patient satisfaction,has the advantages of economy,convenience,safety,effectiveness and so on,which can be promoted for rehabilitation treatment postoperative. |
