| Purpose This paper made a systematic study of the literature on TCM and Western medicine for depression,and analyzed the etiology and pathogenesis of depression in TCM and the western medicine pathological mechanism of depression,studied the relevant factors of Chinese medicine constitution,summarized the relationship between Chinese medicine constitution and depression,and explored the treatment of depression with traditional Chinese medicine,conducted an epidemiological survey on college students in University Town of Huangjiahu,Wuhan,and observed the related influencing factors of depression in college students,made an in-depth study on the distribution characteristics of different constitutions and the relationship between factors such as grade,age,birthplace and the development of depression,Analysed the distribution pattern of TCM constitution of college students ’depression,which provided basis and reference for the prevention of college students ’depression.And Xiaoyao Pill which is a classic prescription for soothing liver and relieving depression was used to intervene in the depression model rats,explored the improvement of depression state of xiaoxiao pill and its mechanism in rats based on BDNF-TrkB pathway,Through the clinically proven Xiaoyao Pill,the scientificity of Shugan Jieyu Method in treating depression is reversed..Methods 1.Theoretical discussion: Consult the literature related to depression in Chinese and western medicine,sort out western medicine pathology research and treatment of the etiology,pathogenesis and depression of TCM depression,explore the physical distribution of TCM in depression and the treatment of TCM.2.Epidemiological Investigation: 2.1 Analysis of the incidence of depressive symptoms and related factors in college students.The incidence of depression in college students and the analysis method of related factors 600 college students in Huangjiahu University Town from September 2015 to September 2017 were selected as the research objects.The application flow called the Depression Self-rating Scale,formerly known as the Center for Epidemiological Survey,Depression Scale(CES-D),to evaluate the depression scores of the enrolled subjects,Depression evaluation standard: total score ≤15 is divided into no depressive symptoms,16-19 points may be depressive symptoms,≥20 is definitely depressive symptoms.According to the evaluation criteria,the incidence of depressive symptoms in college students was investigated and analyzed.The college students who definitely have and may have depressive symptoms(CES-D score> 15 points)were included in the observation group,the normal group were college students without depressive symptoms(CES-D score ≤ 15 points).And investigate its baseline data,using multi-factor Logistic regression analysis method to analyze the risk factors of depression in Huangjiahu University City students.2.2 Analysis of the relationship between different constitutions and depression of college students According to the table of TCM constitution classification and judgment,the constitution classification of Observation College students was investigated.The types of constitution can be divided into Yin deficiency,idiosyncrasy,phlegm and dampness,blood stasis,damp heat,peace,and yang deficiency.,Qi deficiency,Qi depression,you can first calculate the scores of each subscale,that is,the original score = the sum of the scores of each item,and the conversion score of each subscale is 0 to 100 points.After calculating the original score,it can be converted into Conversion score.3.Experimental Study: Twenty-two SPF SD rats of 8 weeks old were completely randomized by random number method and divided into 4 groups: normal group,model group,western medicine group and traditional Chinese medicine group,with 8 rats in each group.The normal group was given intragastric saline,and the remaining three groups were given chronic restraint modeling.The model group was given normal saline,the western medicine group was given fluoxetine,and the Chinese medicine group was given Xiaoyao pills.Body weight measurement,open box experiment measurement,sugar water consumption experiment measurement and food consumption experiment were performed before modeling,after modeling and after administration.Western Blotting technology was used to detect the expression of BDNF and TrkB protein in rat hippocampus;RT-PCR technology was used to detect the expression of BDNFmRNA and TrkBmRNA in rat hippocampus.SPSS 19.0 was used to perform statistical analysis on the obtained data to explore the effect of Xiaoyao Pill on improving the depression behavior of depression model rats.Results 1.Qi stagnation is a susceptible constitution for emotional disease syndrome and prone to depression.Depression patients with liver qi stagnation and liver depression and spleen deficiency are most common.Therefore,in the current clinical treatment of depression,the method of relieving liver and relieving depression is mostly adopted,and the effect of Xiaoyao Pill on depression has been clinically verified.2.Investigating the incidence of depression among college students: A survey of 600 college students in Huang Lake University City was conducted.Finally,585 cases were included in the study,and 15 cases fell off.105 of 585 patients must have depressive symptoms,the incidence of depressive symptoms was 17.95%;350 cases without depressive symptoms(59.83%);130 cases with possible depressive symptoms(22.22%).The survey analysis shows that Urban Family Location,single-parent family,disharmonious interpersonal relationship and worrying about employment prospect were the risk factors of college students ’depressive state.3.The distribution pattern of TCM physiques of college students with depression: in the observation group,the physique frequency from low to high was: 1.70%(4/235)for idiosyncrasy,2.98%(7/235)for phlegm and dampness,3.83%(9 / 235)for dampness and heat,blood stasis quality 3.83%(9/235),peace quality 4.26%(10/235),Yin deficiency quality 6.81%(16/235),Yang deficiency quality 17.02%(40/235),Qi deficiency quality 21.70%(51/235),Qi Depression 37.87%(89/235);the physical frequency of the normal group in descending order is: Qi Deficiency 3.43%(12/350),Yin Deficiency 3.34%(12/350),Special idiosyncrasy 3.71%(13/350),qi stagnation quality 4.29%(15/350),phlegm quality 6.00%(21/350),blood stasis quality 7.43%(26/350),damp heat quality 7.71%(27/350),Yang deficiency mass 11.71%(41/350),and peace mass 52.29%(183/350).The frequency of peace and quality in the observation group was lower than that in the normal group(χ2 = 146.717,P <0.05),and the frequency of Qi depression and Qi deficiency in the observation group was higher than that in the normal group(χ2 = 108.504,48.857,P <0.05).Correlation analysis of TCM constitution and CES-D scores of college students with depression: Correlation analysis showed that there was no significant correlation between CES-D scores of college students and blood stasis,phlegm and traits(P> 0.05),and Qi deficiency,qi depression,yin deficiency,and damp-heat quality were positively correlated(P <0.05),and were negatively correlated with calmness and yang deficiency.4.Before modeling,compared with the control group,there was no significant difference in body weight,total distance and sucrose consumption(P > 0.05).After modeling,compared with the control group,the rats in the model group,the Chinese medicine group and the western medicine group lost weight,the total distance and the preference for sucrose(P < 0.05).After administration,compared with the control group,the weight loss,total distance and sucrose preference of rats in the model group were statistically significant(P < 0.05);compared with the model group,the weight gain,total distance and sucrose preference of rats in the traditional Chinese medicine group and the western medicine group were statistically significant(P < 0.05).Compared with the control group,the expression of BDNF and TrkB decreased in the model group,and BDNF protein decreased in the western medicine group and the traditional Chinese medicine group(P < 0.05).The expression of BDNF mRNA and TrkB mRNA decreased in the model group and the traditional Chinese medicine group(P < 0.05);Compared with the model group,the expression of BDNF and TrkB protein and gene in the western medicine group and the traditional Chinese medicine group increased significantly(P < 0.05).Conclusion 1.Consult the literature related to depression in Chinese and western medicine,analyze the etiology and pathogenesis of TCM depression syndrome and western medicine pathological mechanism and treatment of depression,summarize the relationship between TCM constitution and depression,and clarify that Qi depression is a susceptible constitution for emotional disease syndrome and prone to depression.Depression patients with liver qi stagnation and liver depression and spleen deficiency are the most common.In the current clinical treatment of depression,the method of relieving liver and relieving depression is mostly adopted.2.This study investigated the depression of college students in four universities in Huangjiahu University Town,and the results showed that the rate of depressive symptoms was 17.95%.The risk factors for depression in college students were the location of family,single parent family,disharmony of interpersonal relationship,worry about employment prospect,and the high social support and self-psychological adjustment ability were the protective factors.In the aspect of TCM constitution distribution,In terms of physical constitution types of Chinese medicine,depression students are mainly dominated by qi depression.Correlation analysis showed that CES-D scores of college students were positively correlated with qi depression,qi deficiency,yin deficiency and damp heat,but negatively correlated with the quality of peace and yang deficiency.3.Experimental results of behavioral changes such as weight,appetite,horizontal exercise,and sugar water consumption in chronic restraint stress model rats suggested that the rat depression model was successfully modeled;Xiaoyao pill was found to have antidepressant effects on depression model rats,and Its antidepressant effect may be related to the upregulation of the BDNF-TrkB pathway in the hippocampus of rats,It also suggested the characteristics of the "multi-target" thinking of the antidepressant of xiaoyao pill. |
