Objective: Based on data mining technology,this paper combs and summarizes the ancient and modern relevant literature of infantile enuresis,and discusses the law of acupoints selection and compatibility of Tuina in the treatment of infantile enuresis;Then reconstruct the Tuina prescription of infantile enuresis(insufficient of kidney-qi);A randomized controlled study was used to evaluate the safety and effectiveness of the reconstituted prescription in the treatment of children’s enuresis(insufficient of kidney-qi),in order to provide a more effective Tuina treatment scheme for the clinical treatment of children’s enuresis.Methods:1.Document Research(1)Study of ancient documents: Using the combination of computer retrieval and manual retrieval,and relying on Chinese medical codes and relevant ancient books of traditional Chinese medicine,this paper explores the f syndrome and treatment law of ancient Chinese medicine in the prevention and treatment of infantile enuresis.(2)Study of modern documents: Search and consult the relevant literature on Tuina treatment of infantile enuresis included in CNKI,VF and VIP from the self built database to June 31,2021,and establish the literature database by using Excel,IBM SPSS modeler18.0,IBM SPSS statistics 24.0and other software analyze the literature data from three aspects:frequency analysis,correlation analysis and cluster analysis to explore the acupoint selection law of modern Tuina in the treatment of infantile enuresis.(3)Prescription reconstruction: according to the research conclusions of ancient and modern literature,reconstruct the prescription of Tuina for the treatment of infantile enuresis(insufficient of kidney-qi).2.Clinical research: 96 patients with insufficient of kidney-qi enuresis who met the diagnostic criteria and inclusion criteria were randomly divided into treatment group,control group 1 and control group 2.In the treatment group,the acupoints were Shenjing,Pijing,Baihui(GV 20),Sanyinjiao(SP 6),Shenshu(BL 23),Dantian etc;The manipulation includes tonifying Shenjing,tonifying Pijing,pressing and kneading Baihui(GV 20),Sanyinjiao(SP 6),Shenshu(BL 23),rubbing Dantian etc.The points of control group 1 were Sanguan,Wailaogong,Dantian,Shenshu(BL 23)and Mingmen(GV 5).The manipulation includes pushing Sanguan,kneading the Wailaogong,kneading the Dantian,Shenshu(BL 23)and Mingmen(GV 5).In the control group 2,the acupoints were Errenshangma,Shenjing,Pijing and Ganjing;The manipulation includes kneading the Errenshangma,tonifying the Shenjing,transporting water into the soil and clearing the Ganjing.After one course of treatment(once a day,6 times a week,4 weeks as a course of treatment),the clinical efficacy,safety,compliance and longterm efficacy of the three groups were compared and analyzed.Results:1.Document Research(1)Study of ancient documents: The name of enuresis has been used since the Han Dynasty;The understanding of its etiology and pathogenesis began with the Huangdi Neijing,and then tended to be perfect in the Ming and Qing Dynasties.The common syndrome types include insufficient of kidneyqi,insufficient of kidney-yin,insufficient of temper-qi,deficiency of lung qi and so on;The diagnosis is mostly based on symptoms,and there are also records of enuresis pulse diagnosis,which is mainly distinguished from urinary incontinence;In terms of treatment,before the Han Dynasty,it was mainly warm and tonic.In the Song Dynasty,it was advocated to treat the disease with astringent products.In the Ming Dynasty,it began to emphasize syndrome differentiation and treatment of both symptoms and signs;There were various methods to treat infantile enuresis in ancient times,among which traditional Chinese medicine and acupuncture were the most widely used,and massage therapy,sticking therapy and guidance therapy were also recorded.Guanyuan(CV 4),Dadun(LR 1),Shenmen(H 7),Weizhong(B 40),Taichong(Liv 3)and Qihai(CV 6)are the most commonly used clinica points(2)Study of modern documents:(1)Frequency analysis: The frequency and support of acupoints are as follows: Shenshu(BL 23),Spine,Sanyinjiao(SP 6),Baliao,Baihui(GV 20),Pijing etc;The acupoints on Bladder meridian of Foot-Taiyang,Conception vessel and Governor vessel are often selected.Tuina techniques are complex and diverse,and the pressing and kneading method,pushing method and kneading method are used the most frequently;Children with kidney qi deficiency enuresis are the most documented,and the most commonly selected acupoints are Shenshu(BL 23),Shenjing,Baliao etc;The most commonly used acupoints for infantile enuresis of qi deficiency of spleen and lung type are Pijing,Zusanli(ST 36),Feijing etc;The most commonly used acupoints for infantile enuresis of dampness-heat of liver channel type are Ganjing,Tianheshui,Xiaochang etc.(2)Correlation analysis: the correlation analysis shows that the sequence of high-frequency point pairs is: Pishu(BL 20)→Shenshu(BL 23),Pangguang Shu(BL 28)→Shenshu(BL23);The high-frequency acupoint pair of children with kidney qi deficiency enuresis is Baliao→ Shenjing;The high frequency acupoints of children’s enuresis with qi deficiency of spleen and lung syndrome are Feijing→ Pijing;The high-frequency acupoints of dampness-heat of liver channel syndrome in children with enuresis is Xiaochang→ Ganjing.(3)Cluster analysis: high frequency acupoints can be clustered into 5 categories,and the core prescriptions are Guanyuan(CV 4),Qihai(CV 6),Zhongji(CV 4),Baliao,Baihui(GV 20)etc;Cluster analysis of highfrequency acupoints in children with kidney qi deficiency enuresis can be divided into four categories;Cluster analysis of high-frequency acupoints of children’s enuresis with qi deficiency of spleen and lung syndrome can be divided into three categories;Cluster analysis of highfrequency acupoints of dampness-heat of liver channel syndrome in children with enuresis can be divided into two categories.2.Clinical research:(1)Overall efficacy evaluation: The total effective rate of the treatment group(86.67%)was significantly higher than that of the control group 1(82.76%)and the control group 2(80.00%)(P<0.05).(2)Comparison of bladder volume: there was significant difference in the distribution of bladder volume between the treatment group and the control group(P<0.05);There was significant difference in bladder volume between the three groups(P<0.05).(3)Comparison of clinical symptom and sign scores: there was significant difference in the total score of symptoms and signs and the total score of main symptoms among the three groups(P<0.05),There were significant differences among the three groups in the frequency of enuresis,urine volume,sleep depth,cold shape and cold limbs(P < 0.05).(4)Comparison of long-term efficacy: follow-up 3 months later,the recurrence rate of the treatment group(19.23%)was significantly higher than that of control group 1(37.50%)and control group 2(41.67%)(P<0.05).Conclusions:(1)Literature research: The most commonly used acupoints for the treatment of infantile enuresis with Tuina therapy are Shenshu(BL 23),Spine,Sanyinjiao(SP 6),Baliao,Baihui(GV 20),Pijing and other acupoints with the functions of Tonifying the kidney and Qi,solidifying the body and stopping the sequelae,and paying attention to the selection of acupoints based on syndrome differentiation and along the meridians,In the compatibility of acupoints,the points of this meridian,upper and lower points,front and rear Shu Mu points,etc.Through the ancient and modern literature data mining,the massage prescription for the treatment of infantile enuresis(sufficient of kindey-qi)is reconstructed: tonifying Shenjing,tonifying Pijing,kneading Baihui(GV 20),Sanyinjiao(SP 6),Shenshu(BL 23),moxibustion Dantian,rubbing Baliao and chiropractic.(2)Clinical research: the Tuina prescription based on literature reconstruction has better clinical efficacy than the traditional massage prescription.It can provide a new treatment scheme for the clinical treatment of infantile enuresis,which is worthy of clinical application.
