In clinical practice,accurate collection and understanding of symptoms is the primary prerequisite for precise treatment.Hand and foot cold,as one of the common symptoms in traditional Chinese medicine,has attracted the attention of medical practitioners throughout history for its clinical significance.As hand and foot cold is a typical conscious symptom,its"abnormal feeling of body surface temperature subjectively experienced" caused controversy under the impact of scientism.Many scholars advocate for objective and quantitative research on conscious symptoms.Under the theoretical system of traditional Chinese medicine,does this so-called "objectification" and "quantification" have indicative value for clinical diagnosis and treatment,and can it bring about theoretical innovation?Or is it reasonable for traditional Chinese medicine to experience the cold and hot sensation in patients’ hands and feet,based on the mutual cooperation between doctors and patients,using the diagnostic and treatment method of "four diagnostic methods combined",under the medical theory constructed by the sublimation of medical experience?There is relatively little systematic and comprehensive research on this issue.Based on this consideration,this study establishes the following research objectives and content.1 Research Purpose1.1 Provide a clear understanding and definition of the concept and extension of hand foot cold syndrome;1.2 Develop a systematic understanding and grasp of the formation mechanism of hand foot cold syndrome;1.3 Comprehensively summarize and summarize the differentiation and treatment rules of hand foot cold syndrome;1.4 Clear sorting and identification of the historical origin and era characteristics of hand and foot cold syndrome;1.5 Summarize the rationality and characteristics of the diagnosis,treatment,and differentiation of hand and foot cold in traditional Chinese medicine;1.6 Think about the conscious symptom of cold hands and feet from the perspective of iconography.1.7 Think about the conscious symptom of hand foot cold from the perspective of iconography.2 Research MethodsThis study mainly uses six research methods:literature review,systematic classification and organization,vertical and horizontal connections,text analysis,case analysis,and data analysis.2.1 Literature Traceability MethodIt is a research method that traces the background knowledge of social,political,technological,economic,and cultural development related to hand and foot cold syndrome from different eras,in order to make theoretical analysis and era characteristics analysis of relevant literature materials influenced by social,technological,and cultural characteristics of different historical periods.2.2 System classification and organization methodIt is a research method that categorizes and organizes the relevant academic content and the ideas contained in the syndrome of foot cold according to two different academic schools and viewpoints in different historical periods and the same period.2.3 Vertical and horizontal connection methodIncluding two methods:horizontal connection and vertical connection.The horizontal connection method is a research method that compares and displays literature related to hand and foot cold under the influence of different theories in the same historical period;The vertical connection method is a research method that combines the academic characteristics of relevant literature and materials from different historical periods.2.4 Text analysis methodThe method of text analysis is mainly applied in two aspects.One is the interpretation and analysis of the academic viewpoints of different schools of thought in Western modern philosophy and traditional Chinese medicine.The second is the elucidation and analysis of the different academic propositions of doctors who integrate traditional Chinese and Western medicine.2.5 Case AnalysisConduct specialized research on representative medical thoughts and academic propositions related to hand and foot cold in medical works,with the aim of summarizing and simplifying the research.2.6 Data analysis methodThe data analysis method is mainly aimed at a large number of prescriptions,and through the use of frequency statistics,association rules,and community analysis methods,it provides a clear and intuitive presentation of the disease types,syndrome types,concurrent symptom groups,basic information of prescriptions,and other related diagnosis and treatment laws and medicinal characteristics of hand and foot cold syndrome.3 Research results3.1 Regarding the cognition and understanding of hand and foot cold syndrome,the Qin and Han dynasties and their previous periods were the period of its theoretical germination and initial formation,the Jin and Northern and Southern Dynasties were the period of its theoretical enrichment,the Sui,Tang,and Five Dynasties were the period of its theoretical consolidation,the Song,Jin,and Yuan dynasties were the period of its comprehensive development and debate,the Ming and Qing dynasties were the period of its blooming and steadfast adherence,the modern era was the period of its theoretical evolution,and the modern era was the period of its theoretical innovation.3.2 The definition of the concept of hand and foot cold should be:hand and foot cold,symptom name,refers to the feeling of cold and discomfort in the hands and feet that the patient experiences on their own,and can often be detected through the use of palpation methods by doctors.It is also common to see signs of lower than normal body surface temperature range values measured by medical staff assisted thermometers and other temperature measurement methods.The extension of concept extension mainly includes two aspects:the range and degree of hand and foot cold.3.3 The mechanism of the occurrence of hand and foot cold syndrome can be divided into three parts,and its understanding can be approached from the disease location on the surface and inside.The disease is located on the surface,and there is a difference between the external pathogenic factors that block the meridians and meridians of the body and joints;The location of the disease belongs to the internal,and there is a difference between the deficiency and hyperactivity of visceral function.3.4 The differentiation and treatment methods for hand and foot cold syndrome can be divided into two categories:internal treatment and external treatment.Internal treatment method:For those with superficial qi stagnation,sweating can relieve it;For those with yang deficiency and yin excess,warm and replenish them;For those who lack nourishment due to blood deficiency,supplement and regulate them;Those with internal obstruction of evil heat may clear or attack;Those who accumulate blood stasis internally can dissipate blood stasis and regulate it smoothly;For those with stagnation of yang qi,they should be relaxed and reconciled;Phlegm turbidity obstructs the chest,causing vomiting and rising;Those who block yang stagnation due to dampness can dispel dampness and promote it.External treatment methods:mainly include moxibustion,ironing,immersion,acupuncture,and guiding methods.3.5 There are many types of diseases with cold hands and feet as the main symptom,which can be seen in both severe and even fatal diseases,as well as chronic debilitating diseases.It involves a wide range of traditional Chinese medicine syndrome diagnoses,mainly characterized by symptoms of yin,cold,and deficiency.The common symptoms of hand and foot cold mainly involve four categories of symptoms:"diet or taste symptoms","bowel and fecal symptoms","pain symptoms",and "mental or emotional symptoms".According to the classification of symptoms in various departments,the common symptoms are mainly symptoms of heart,spleen,stomach,and kidney disease in internal medicine,as well as gynecological and pediatric symptoms.The formulations used for treating hand and foot cold are relatively comprehensive,including decoction,powder,pill,and pill.The classification of formulas involved is also relatively comprehensive,with the application of warm tonics being the most widespread.The prescription for treating hand and foot cold as the main symptom has a wide range of medicinal flavors and involves a comprehensive range of drug functions.However,the prescription is mainly used for warming the internal circulation,tonifying qi,and regulating qi.3.6 Symptoms of hand and foot cold can be classified and quantified.Before quantification,individual factors such as gender,age,physical condition,emotional state,and external factors such as regional environment and climate must be considered to have a significant impact on the formation of hand and foot cold.When quantifying,first of all,it is necessary to consider and grasp the types of diseases and their symptom groups seen in the foot cold syndrome;Subsequently,the judgment of mild,moderate,and severe foot cold syndrome is mainly based on the patient’s conscious perception,degree of cold,range,frequency,duration,accompanying symptom groups,and relief factors.3.7 From the perspective of phenomenology,back to the "cold hand and foot disease" itself,as a "sense of reality",the abnormal feelings of patients or doctors from the body surface temperature of hands and feet are realistic and rational.Humans use touch to perceive the surface temperature of other people’s hands and feet.At the same time,others make true and accurate judgments about the sensory nature of body surface cold and heat based on tactile experiences originating from the world and other oneself.4 Research Conclusion4.1 The understanding of hand and foot cold syndrome is not achieved overnight,but gradually improved through the infiltration of medical thinking at different times in the long historical process.4.2 In traditional Chinese medicine,the systematic construction of relevant theories surrounding hand and foot cold has become systematic and comprehensive.4.3 As a conscious symptom of hand and foot cold,the diagnosis and treatment methods of traditional Chinese medicine are reasonable and can intuitively reflect the essence of the symptoms.Quantitative research is only applicable to grading studies such as the degree of cold.4.4 On the premise of accurate qualitative research,the study of hand and foot cold can be classified and quantified.
