| Objective: To discuss the nursing effect of iontophoresis combined with five elements finger exercises in the treatment of patients with rheumatoid arthritis with cold and wet paralysis,and to provide theoretical and practical basis for its clinical application.Methods: In this study,90 inpatients meeting the diagnostic criteria of rheumatoid arthritis in the Rheumatology and Immunology Department of a Grade-A Traditional Chinese Medicine Hospital in Hunan Province were selected by convenient sampling method.90 patients were randomly divided into control group A(conventional nursing + TCM iontophoresis),test group B(conventional nursing +TCM five-element finger exercises)and test group C(conventional nursing + TCM iontophoresis + TCM five-element finger exercises)with 30 cases each.After one course of intervention,the differences of TCM syndrome score,TCM syndrome score,hand grip strength,quality of life score(HAQ),functional impairment signal score(SOFI),visual analog scale(VAS),erythrocyte deposition rate(ESR),rheumatoid factor(RF)and C-reactive protein(CRP)levels among the three groups were observed.To analyze the effects of different interventions on the clinical symptoms of rheumatoid arthritis and the degree of remission of inflammation.SPSS22.0 was used for statistical analysis of data,ANOVA was used for statistical inference of measurement data,LSD method was used for pound-wise comparison between groups,and rank sum test was used for analysis when variance was uneven.Counting data were statistically analyzed by Chi-square test,and the test level was P < 0.05.Results:(1)Baseline data: When comparing demographic data such as gender and age of patients and disease-related data such as years of diagnosis and severity of disease among the three groups,there was no statistical significance(P > 0.05),indicating that the data were comparable.(2)TCM syndrome integral efficacy: After intervention,the total effective rate of control group A was 80.0%,that of trial group B was 63.3%,and that of trial group C was 90.0%.Chi-square segmentation showed that there was a statistically significant difference between group C and group B(P < 0.017).(3)TCM syndrome score: After intervention,the TCM syndrome score of control group A was decreased by 5.43±2.42 points on average,that of test group B was decreased by 6.23±3.55 points on average,and that of test group C was decreased by8.70±3.30 points on average,and the improvement of test group C was better than that of control group A and test group B(P < 0.05).(4)Indicators of hand function activity:(1)After intervention,SOFI scores of hand function impairment signal in control group A decreased by an average of 2(1,2)points,in test group B decreased by an average of 3(2,3)points,and in test group C decreased by an average of 3(3,4)points.The improvement of test group C was better than that of control group A and group B,and the improvement of test group B was better than that of control group A(P < 0.05).(2)After intervention,the grip strength of the three groups was significantly increased(P < 0.05).The grip strength of control group A was increased by an average of 5.88±3.15 points,that of trial group B was increased by an average of 6.03±2.05 points,and that of trial group C was increased by an average of 11.34±5.04 points,and the improvement of trial group C was better than that of control group A and trial group B(P < 0.05).(5)Quality of life indicators:(1)The HAQ scores of the three groups were significantly decreased after intervention(P < 0.05).Control group A decreased by an average of 4(4,5)points,test group B decreased by an average of 3.5(3,4)points,and test group C decreased by an average of 7(6,7.25)points,and the improvement of test group C was better than that of control group A and test group B(P < 0.05).(2)VAS scores of the three groups were significantly decreased after intervention(P < 0.05),with an average decrease of 2(1,2)points in control group A,1(1,2)points in test group B,and 2(2,3)points in test group C.The improvement effect of test group C was more significant than that of control group A and test group B(P < 0.05).(6)Hematological indicators: After intervention,the average RF of group A in control group was decreased by 51.45(18.55,237.58)IU/m L,the average RF of group B was decreased by 8.60(-5.40,83.08)IU/m L,and the average RF of group C was decreased by 116.90(10.85,270.90)IU/m L.RF levels in control group A and test group C were significantly decreased(P < 0.05),there was no statistical significance in RF levels in test group B before and after intervention(P > 0.05),and RF improvement in test group C and control group A was better than that in test group B(P < 0.05).(2)After intervention,CRP levels in the three groups were significantly decreased(P < 0.05),with the mean decrease of 10.40(4.11,16.17)mg/L in control group A,15.76(7.02,27.00)mg/L in test group B,and 20.26(7.28,29.08)mg/L in test group C.There was no significant difference among the three groups(P > 0.05).(3)After intervention,ESR levels in all three groups were significantly decreased(P < 0.05).ESR in control group A was decreased by 12.50(4.50,23.25)mm/h,in test group B by 17.85(9.00,27.50)mm/h,and in test group C by 31.25(14.00,52.50)mm/h.The improvement of test group C was better than that of control group A and test group B(P < 0.05).Conclusion: The combination of Chinese medicine ion import and Chinese medicine five elements finger exercises can significantly improve the nursing effect of patients with rheumatoid arthritis,reduce TCM syndrome score,improve hand function,reduce hematology index and improve life quality. |
