Investigation On Community Mental Health Service In Changning District Of Shanghai

BackgroundSince the1950s, the World Health Organization worldwide has advocated deinstitutionalization movement in the field of psychiatric treatment, promoting the medical services outside the hospital, improving mental social function of patients in a familiar atmosphere in the community so as to help them return to normal society. Countries carry out some variety of modes to explore. Our community mental health is limited by the large number of patients, a serious shortage of mental health resources and other factors, has developed more slowly. Domestic experts suggest, under the guidance of the professional medical community, relies on more resources to develop community mental health services and improves the community service system.Around this background, the community health service centers belonging to Changning District in Shanghai have taken the lead to practice in the public mental health and provide community mental health services since the second half of2009.ObjectiveTo analyze the mental health mode and service efficiency in Community Service Center belonging to Changning District, provide advices and strategies to adjust relevant policies and improve efficiency.Specific objectives are to described the community mental health service model in Changning District; to comparative analysis community mental health services status in different periods; from the service provider both the demand side (the main guardians with schizophrenia patients) of the existing state of community mental health services, awareness rate and satisfaction investigations; to provide some recommodations for improving community mental health services.Material and MethodsIn this study, data information including the daily reports and survey data, daily reports is from the region of the data file, annual summary report of six quality control,precision anti-database data, statements of work and so on. Survey data from two self questionnaires:conduct of service providers and the demand side (the main guardians with schizophrenia patients), analysis the model of community mental health services, human resources, awareness rate, and so on. Chi-square test was used to compare the rates or proportions and Rank sum test was used to compare significiant difference among different groups.ResultsChangning District established three-level community mental health service network and medical personnel from the Community Service Center provided basic mental health services. Analyze the mental health service status from2009to2010in Changning District.Human resources and equipment:compared with2009, community mental health service providers in2010increased by135%, month Average working hours in2010increased by60%; work performance:in2010, community mental health services covered has certain advantages in three fields such as Population coverage,services and efficiency; awareness rate:Evaluate community service provider in2010on knowledge of mental health awareness among the general population to reach78.9%, higher than the general population survey,65%standard.service providers working knowledge the major problems faced by service providers in2010were family members, lack of patient identity (64.5%), lack of professional knowledge (52%), lack of professional guidance received (50%). Cognitive demand side:the families of503patients with schizophrenia receiving community mental health services reached72.4%, overall satisfaction to86.9%.Conclusions and RecommendationsWe proposed to strengthen the scientific management and increase staffing of community mental health services and adjust the regional health policies and carry out specific operational guidance.Meanwhile, we proposed to strengthen mental health awareness to improve the satisfaction of all parties.In conclusion, Community Health Service Center belonging to Changning District provided a well-developed chance for community mental health services, but there are many problems need to be resolved nowadays.
