Observation On Curative Effect Of Yiqi Jiedu Tongluo Method On Primary Biliary Cirrhosis

Objective:1 This study was to observe the clinical curative effects of three treatments of primary biliary cirrhosis (PBC), which are UDCA, the method of Yiqi Jiedu Tongluo(Chinese medicine) and the combination of the two methods. Then the writer evaluated three aspects of these treatments including symptoms and signs, physicochemical and imaging examination, and the adverse events.2 The study provides evidence-based basis to establish the standardized clinical programs about the treatment of PBC in Chinese medicine.Methods:51 patients with PBC, who came from Dongzhimen hospital of Beijing University of Chinese Medicine in the period of 2013.9 to 2015.1, were selected and randomly divided into the UDCA group(n=15),the Chinese medicine group(n=18) and the combination of UDCA with Chinese medicine group (n=22). The UDCA group was treated with UDCA with Silibinin Capsules; Chinese medicine group was treated with Yiqi Jiedu Tongluo combined with Silibinin Capsules; and the combination group was treated with Yiqi Jiedu Tongluo, UDCA and Silibinin Capsules. Three groups were observed for 6 months. Assessments were carried according to situations about the PBC patients at the time of before-treated, three-month-treated and six-month-tread, which included clinical signs and symptoms,TCM syndrome score, liver function, liver ultrasound and adverse reactions. This study uses SPSS17.0 statistical software for statistical analysis. P<0.05 was considered statistically significant.Results:1 nalysis after Chinese medicine treatmentAccording to the recovered degree, the writer divided four levels:recovered, effective, valid and invalid. After six months’ treatment, the number of the UDCA group at the four-level were respectively 0,0,4 and 11. The total effective rate was 27%. The number of the Chinese medicine were 6,7,5 and 0; and the total effective rate 100%. About the UDCA with Chinese medicine group were 3,13,2 and 0; and its total effective rate was 100%. What’s more, the curative effects of traditional Chinese medicine and UDCA with Chinese medicine group were stronger than the UDCA group. Chinese medicine and UDCA with Chinese medicine group by X2 test, P> 0.05, the difference has no statistical significance indicating that the effects of these two treatments were quite fair.2 Changes in liver functionThere was statistical significance before and after treatment among three groups. By comparing AST, ALP, TBiL and DBiL they had the same statistical results. The results were both P> 0.05, and the difference had no statistical significance no matter the treatment time was three months or six months; while the ALT and GGT statistical results were both P <0.05(the same) and the difference had statistical significance after six months’ treatment. According to the average rank of the three groups the results of ameliorative effect was:the combination group> the UDCA group> the Chinese medicine group. After six months’ treatment, the number of the Chinese medicine group about recovered markedly, valid, and invalid were 6,5 and 7 respectively. The total effective rate was 61.11%. The number of the combination group were 4,8 and 6. The total effective rate was 66.67%. The UDCA group were 4,4 and 7. The total efficiency of 53.33%. By X2 test three groups’ P= 0,834(> 0.05). There was no different statistical significance. From the above description, it could be seen that traditional Chinese medicine can improve liver function better, but it is much better with the joint of UDCA.3 Ultrasound changesThrough comparation within each group and between groups, liver ultrasound were all P> 0.05, there was no different statistical significance. This illustrated that there was little change in ultrasound among three groups after 6 months of treatment. And the treatment effect is not obvious. What led this might be associated with the small number of samples and the limited treatment time.4 Tongue condition’s changesThrough statistical frequency, the tongue of patients were dark red before any treatment in three groups, which number were respectively 77%,83% and 73% with yellow and white thick greasy coated tongue commonly, which percentages were 83%,77% and 80%. After 6 months’ treatments, the percentages of three groups’ dark red tongue changed to 56%,56% and 87% with thick and greasy fur changed to 61%,56% and 87%. It showed that the Chinese medicines and the UDCA with Chinese medicine group could decrease the symptom. This stated that the UDCA group’s patients had no obvious change, while the Chinese medicine and the UDCA with Chinese medicine groups’ quite improved. However, there were still less than half of the patients’ tongue and furred tongue which did not change significantly when they compared with the previous. It showed that the disease condition was complicated requiring long-term treatment.5 Pulse condition’s changesPulse condition of patients mostly appeared xuan, chen, xi, hua, shu, and ruo to show that the decease mixed deficiency and excess. After six months’ treatment, patients’ pulse condition in Chinese medicine group and the UDCA with Chinese medicine group had been improved much than before. However the UDCA group did not. To sum up, the patient’s Qixue had been changed after taking medicine, shire-duxie had been cleaned up, while UDCA does not have this effect.6 Major adverse eventsDuring treatment 2 patients got rash and relieved to disappear by itself. Two cases of patients felt dizzy possibly by drug overdose. After adjustment patients’ dizziness disappeared. There were no abnormalities when their electrocardiogram, heart rate, blood pressure, urine, stool and routine were checked.Conclusion:1 Patients’clinical signs and symptoms were improved obviously by the method of Yiqi Jiedu Tongluo. And with UDCA could not help to improve patients’these conditions better.2 Among PBC indicators, the signs and symptoms of cirrhosis of the liver function could be relieved apparently by the method of Yiqi Jiedu Tongluo. If it was with the UDCA cirrhosis of the liver function the treatment effect was better.3 The results of liver ultrasound could not be significantly improved by the method of Yiqi Jiedu Tongluo.4 Patients’TCM tongue coating and pulse condition were better with Chinese medicine method of Yiqi Jiedu Tongluo.5 Patients had no significant adverse reactions, and the application security is fine with the method of Yiqi Jiedu Tongluo.
