| BackgroundIn recent years, increasing year by year, patients with diminished ovarian reverse increasingly brought to the attention of the experts at home and abroad. The disease pathogenesis is not clear, no effective medicine symptomatic treatment drugs, most of the use of hormone replacement therapy. Acupuncture treatment of diminished ovarian reserve is in its infancy, most intervention method of clinical research is traditional Chinese medicine with acupuncture and western medicine with acupuncture treatment, but the report of pure acupuncture treatment is fever, and the function of acupuncture and moxibustion on the improvement of the ovarian reserve is not clear.ObjectResearch the clinical curative effect of electric acupuncture on the improvement of the ovarian reserve for patients of diminished ovarian reserveMethodThe selected cases diagnosed with diminished ovarian reserve in Department of Acupuncture, Guang’anmen Hospital, China Academy of Chinese Medical Science from 2014.1 to 2015.3, were a total of 35 cases, all with a elect-acupuncture treatment. Observe and record the patients’follicle-stimulating hormone (FSH), luteinizing hormone (LH), follicle promote hormone/luteinizing hormone (FSH/LH), estradiol (E2) levels during 2-5 days menstruation. Combine menstrual status, serum female hormone and accompanying symptoms, and analysis the clinical curative effect of improving ovarian reserve function.Results(1) Serum level of sex hormoneTreatment for 4 weeks compared with baseline, serum FSH level declined by 6.84 ±10.60 (95% CI: 3.24-10.44, P=0.000), serum LH level by 1.43 (4.85) (95% CI:-1.54-2.21, P=0.175), and FSH/LH ratio by 0.84±2.23 (95% CI: 0.05-1.64, P= 0.037), and serum E2 levels increase by 2 (78.4) (95% CI: -77.90-0.31, P=0.336). Elect-acupuncture treatment of 4 weeks can obviously reduce serum FSH level and FSH/LH ratio, and to reduce serum LH level, elevated serum E2 level effect is not obvious.Treatment for 8 weeks compared with baseline, serum FSH levels declined by 5.53 ±11.34 (95% CI: 1.05-10.02, P=0.018), serum LH level by 2.57 (4.64) (95% CI:-1.09-3.68, P=0.020), and FSH/LH ratio by 0.25±2.15 (95% CI: 0.603-1.10, P=0.553), serum E2 levels rise by 8.53±52.13 (95% CI: -29.15-12.10, P=0.160). Elect-acupuncture treatment can obviously reduce serum FSH, LH levels, and to increase serum E2, to reduce FSH/LH levels effect is not obvious.Treatment for 12 weeks compared with baseline, serum FSH level declined by 7.39 ±9.78 (95% CI: 2.68-12.10, P=0.004), serum LH level by 1.90±4.26 (95% CI: 0.15-3.96, P=0.067), and FSH/LH ratio by 0.72 (2.0) (95% CI: 1.23,1.23, P-0.198), serum E2 levels rise by 2.0 (82.13) (95% CI: 113.40 39.52-), P=0.629). Treatment of electric acupuncture after 12 weeks can obviously reduce serum FSH level, and to reduce the level of serum LH and FSH/LH ratio, to increase serum E2 level is not obvious.Follow-up period compared with the baseline, serum FSH level declined by 5.56 ±5.11 (95% CI: 2.13-8.99, P-0.005), serum LH level by 1.50±1.80 (95% CI: 0.29 2.71-, P=0.020), and FSH/LH ratio by 0.0069 ± 1.59 (95% CI: -1.06-1.07, P-0.989), serum E2 level increased byl (25) (95% CI:-14.62-44.38, P-0.423). Elect-acupuncture therapy after 12 weeks can reduce the serum FSH and LH level, which can sustain 12 weeks.(2) Menstrual conditionsTreatment for 4 weeks compared with baseline, menstrual cycle shorten by0 (3) (P=0.554), menstrual days increased by 0 (1.5) (P=0.278), menstrual quantity increased by 0 (10) (P=0.060).The effect of elect-acupuncture treatment for 4 weeks for shortening menstrual cycle, increasing elevated menstrual days and increasing menstrual quantity is not obvious.Treatment for 8 weeks compared with baseline, menstrual cycle shorten by0 (5) (P=0.755), menstrual days shorten by 0 (2) (P=0.062), menstrual quantity increased by10 (2) (P=0.022).The effect of elect-acupuncture treatment for 8 weeks for shortening menstrual cycle and increasing menstrual days is not obvious, and for increasing elevated menstrual quantity is obvious.Treatment for 12 weeks compared with baseline, menstrual cycle increased by0 (8) (P=0.284), menstrual days shorten by0 (2) (P=0.060), menstrual quantity increased by 16.32 ± 16.06 (P=0.022). The effect of elect-acupuncture treatment for 12 weeks for increasing menstrual cycle and shortening menstrual days is not obvious, and for increasing menstrual quantity is obvious.Follow-up period compared with the baseline, menstrual cycle increased by0 (5) (P=0.726), menstrual days increased by0 (1) (P=0.558), menstrual quantity increased by21.82 ± 8.74 (P=0.000). Elect-acupuncture therapy after 12 weeks can increase menstrual quantity, which can sustain 12 weeks.(3) Emotional symptomsTreatment for 4 weeks compared with baseline, the score of irritability shorten byO (0.5) (P=0.046), the score of depression by 0(0) (P=0.458).The effect of elect-acupuncture treatment for 4 weeks for improving irritability is obvious, and for improving depression is not obvious.Treatment for 8 weeks compared with baseline, the score of irritability shorten byO (1) (P=0.012), the score of depression by 0 (0) (P=0.197).The effect of elect-acupuncture treatment for 8 weeks for improving irritability is obvious, and for improving depression is not obvious.Treatment for 12 weeks compared with baseline, the score of irritability shorten byO (1)(P=0.016), the score of depression byO (0)(P=0.480).The effect of elect-acupuncture treatment for 8 weeks for improving irritability is obvious, and for improving depression is not obvious.Follow-up period compared with the baseline, the score of irritability shorten byO (1) (P=0.102), the score of depression byO (0) (P=0.197).The effect of elect-acupuncture treatment for 8 weeks for improving irritability and depression can not sustain 12 weeks.Does not appear acupuncture adverse events and severe adverse events during treatment patients.Conclusion(1) The elect-acupuncture treatment can reduce the serum FSH level, FSH/LH ratio, can improve the menstrual account, can improve ovarian reverse function, and can adjust the according symptom irritability.(2) Durin g the treatment, patients never appear the acupuncture adverse events and severe adverse events occurred, and the security of elect-acupuncture treatment is well.(3) Liu zhi-shun director’s clinical experience for treating DOR is as follows. Selected acupoint is zhong liao, zi gong, tian shu, guan yuan, and each treatment for 20 minutes,5 times a week during the first four weeks of treatment, three times a week later, on alternate days at a time,8 to 12 weeks of treatment. |
