| IMPORTANCE:Chronic cough is characterized by unexplained cough lasting at least 8 weeks and normal X-ray chest films.It poses a great diagnostic challenge due to myriad etiologies so that various examinations are in need.Long-term recurrent chronic cough and frequent treatment have affected patients’ life and work leading to financial loss and a low life quaility.Chronic cough is regarded as endogenous cough in TCM(traditional Chinese medicine).There is proof that TCM plays a positive role in the prevention and treatment in chronic cough.Modern literature pays more attention to lung-yin deficiency chronic cough.However,there are many cases with lung-yang deficiency syndrome not only in clinical practice but also in TCM’s basic theories and documents. Therefore,studies of lung-yang deficiency chronic cough are necessary.OBJECTIVE:To investigate the etiology,pathology and symptomatology of lung-yang deficiency chronic cough.DESIGN, SETTING, AND PARTICIPANTS:In this retrospective analysis, all medical records for patients seen at respiratory department of Dongfang Hospital affiliated with Beijing University of Chinese Medicine from July 2012 through October 2014 were searched for relevant clinical manifestations. Patients with lung-yang deficiency chronic cough (cough of >4 weeks’duration) were selected for study; 79 patients met the inclusion criteria and were selected for analysis.RESULTS:Among the 79 included patients, femals are four in five and males are one in five.In most cases,recurrence was not concerned with seasons.68 patients were sensitive, in varying degrees, to various allergens like cold air,dust,lampblack,also to speaking,eating and common cold.53.2% patients had normal sense of tastes. Dry mouth is the most common in cases with abnormal sense of tastes accounting for 38.0%.77.2% patients had nomal drinking behavior.13 patients had dry mouthes with no desire to drink.53.2%patients had normal defecation. While 21 patients had loose stools,9 patients had dry stools and 7 patients had sticky stools.Most patients’urination was normal.In lung-yang deficiency syndrome cases,88.6%patients had cold backs,48.1% had cold stomaches,57.0%had chilly sensation,10.1% had cold hands and feet,5.1% had cold waists,36.7%had whitish furs,22.8% had thready pulses and 45.6% had slippery pulses.Other accompanying symptoms could be seen.44.3% patients had syndrome of dampness-heat blocking the lung,24.1% patients had lung and spleen-yang deficiency syndrome and 44.3% had syndrome of wind evil encroaching the lung.CONCLUSIONS AND RELEVANCE:The main location of lung-yang deficiency syndrome is lung and often related to spleen and kidney in the process of disease.Its pathogenesis is lung-yang deficiency,phlegm retention, and Lung Qi upward reversal.And a cold back is the typical clinical manifestation of lung-yang deficiency chronic cough. |
