Use the Method of Health Education to Construct the Mode of Community Health Management of Bone and Joint DiseaseAbstract:Bone and joint disease is a common disease, chronic disease, representative of senile diseases, Recently, the growth trend of the incidence and morbidity of it has threatened our health further. The construction of Community Healthy Management Mode of bone and joint disease connects risk factors with the idea of Healthy Management. We also use the conjunction point of health management concept and content of community health services to construct the model. What’s more, it seeks for approaches which are practical and effective to preventing and controlling bone and joint disease,which also creates a basic platform for disease prevention and control.As a first-step exploration of step-by-step application in the health system of our nation, Construction of bone and joint disease healthy management model is not only a supplement to the traditional Chinese diagnosis and treatment patterns, but also a necessity to the improvement of health care system in China.The research is divided into four parts:The first part mainly introduces the definition, diagnosis, treatment, epidemiology, risk factors of bone and joint disease. The risk factors are split into three types: modifiable ones, potentially modifiable ones and immutable ones. To remove or reduce the reversible factors to delay the progress of the disease is the core target of this research.The second part takes the ancient health management thoughts as the first point, resuming the connotation and mode of health management, the relationship between health management and health education. Further, it sketches the current utilization of health management in China. At the same time, it uses some cases to analyze the application of health management in the research of preventing bone and joint disease, and introduces the title of how to construct the community health management model of bone and joint disease.The third chapter discusses the importance of health education in health management from the micro level and adopts the empirical method to study the osteoarthrosis education demand of a community in Nanjing, in order to provides the theoretical basis on regulate the behavior of community health education and health management. The fourth part mainly expounds the subject, the main contents, the methods and related policy of constructing the community health management model of bone and joint disease. The executive body includes health management team, home health care, the patients’ himself (herself). The medical process of health management consists of three basic components:health information collection, information screening and assessment, health intervention. The management methods have hierarchical management, contract management, family-community interaction management.
