| Purpose:1.Analyze the literature on treating scapulohumeral periarthritis with themethods of Traditional Chinese Medicine (TCM), summarize the traditionaltherapiesã€types based on syndrome differentiationã€selection of acupoints andinstrumentsã€needling methodsã€specific manipulationã€effect and other aspects,and study the current situation of treatment for scapulohumeral periarthritisat the present stage.2.According to the theory of meridian tendons, functional physiology, andstandard positions released by Muscle Tendon Theory and Clinical pain DiagnosticScience, there are6lines for meridian tendons to circulate in shoulder-back,they are three yang meridian tendons of hand, and three yin meridian tendonsof hand, then confirm16positive A-shi acupoints by palpating shoulder-backthe meridian tendon circulating and acupuncture them. Confirm (levator muscleof scapulaã€rhomboideusã€cucullarisã€musculus supraspinatusã€infaspinous muscleã€musculus teres minorã€musculus subscapularisã€trigonal muscleã€biceps brachiiã€musculus triceps brachii), as the muscle group for stretching treatment, theseparts by palpating according to functional physiology and combine acupuncturewith stretch for treatment. The clinical effectã€range of motionã€pain intensityã€physical function and other aspects of treatment for scapulohumeral periarthri-tis with the method of acupuncture combined stretch are assessed by a randomizedcontrolled study, and the assessment is objective, which can prove that themethod of acupuncture combined stretch is superior in clinically treatingscapulohumeral periarthritis.3.Provide new direction for study of acupuncture, change the single treatmentin clinical acupuncture and moxibustion, prove the comprehensive therapy of TCM better in clinical treatment, and supply the comprehensive therapy with clinicalbasis as well as theories.Material and method:87casesofclinicallyprovenscapulohumeralperiarth-ritis, in accordance with the random number table, were divided into3groups:A.29cases for acupuncture; B.28cases for stretch; C.30cases for acupuncturecombined with stretch, group A was cured by acupuncturing the A-shi acupointsalong meridian tendons, group B was treated by stretching, group C applied themethod of acupuncture combined stretch. The clinical effectã€different degreesof painã€functions of articulatio humeriã€different active levels of articulatiohumeri of pre and post treatment were assessed and analyzed.(1) Group A(acupuncture): Applied the method of acupuncture.acupointsselection: selected Sub-SI9ã€Sub-SI10ã€Sub-Shoulder Pain Pointã€Sub-SJ13ã€Sub-SJ14ã€Sub-SI11ã€Sub-SJ15〠Sub-LI15ã€Sub-SI12ã€Sub-SI13ã€Sub-GB21ã€Sub-LU3ã€Sub-Jian nei lingã€Sub-LU1ã€Sub-Ju jianã€Sub-Qing ling. acupuncture apparatus:Huatuo brand disposable sterile fine needle (specification:0.30mm×50mm). operation method: the affected side of shoulder was fully exposed with sittingposition or lateral recumbent posture, and marked the above acupoints with makerpen, regularly disinfected the acupoints with75%alcohol without omissions,and the hand of operators were disinfected with iodine and take off the iodinewith75%alcohol, then put the left thumb on the acupoints and accuratelypenetrated the acupoints with needles held in the right hand, reforcing andreducing after developing needle sensation, retaining needles for30minutesand manipulating needles at each10minutes.(2)B group (stretching group):adopting stretching therapy treatment.(1)Muscle group: levator scapulaeã€rhomboideusã€trapeziusã€supraspinatusã€M.infraspinatus and M.teres minorã€musculus subscapularisã€deltoidã€bicepsbrachiiã€triceps brachii(2)relaxing before operation:the patients need to sitdown on the chair and the docdtors need stand the affected side of patients,andadopting mild manipulations (such as graspingã€pressure and kneading manipulation) for selecting muscle group by using thumbã€index finger and palmroot in order to relax muscle group and relieve muscular tension.(3)stretchingoperation: the operation is same as the method used in Group B mentioned in thebody part.(3)C group(Needling through the muscle tendons A-shi acupoints plusstretching therapy group):adopting needling combined with stretching therapyto treat.Selecting needling pointsã€operation method and stretching operationis the same with A group and B group respectively.(4)The patients in all groups pay more attention to protecting shoulder jointin the process of clinical treatment and avoiding attacking wind-cold pathogenand trauma injuries. Patients should do more functional exercises duringtreatment.the frequent and time is once a day,ten days as a course and willhave clinical functional assessment after two courses.Results:1. Needling through the muscle tendons A-shi acupoints plus stretching therapygroup is superior to needling through the muscle tendons A-shi acupoints onlyand stretching simply.(P<0.05)2.Using ANOVA test to compare shoulder joint pain degree before treatment andthe difference were not significant;comparing the third group of shoulder jointpain scores after treatment with group A and group B,and it has significantdifference has significant difference.(P<0.05);comparing A group with B groupis no different.(P>0.05)3. Using ANOVA test to compare shoulder joint function comprehansive scoresbefore treatment and the difference were not significant(P>0.05); Using ANOVAtest to compare shoulder joint comprehensive scores after treatment,there issignificant difference C group between A group and B group(P<0.05),comparingA group with B group is no different.(P>0.05)Conclusion:1.Needling through the muscle tendons A-shi acupoints plus stretching therapy totreat Scapulohumeral Periarthritis is safeã€simple and practical and effectiveand it will provide a new theory and clinical basis for treating ScapulohumeralPeriarthritis in traditional chinsese medicine comprehensive therapy.2.The efficacy of the treatment group of Needling through the muscle tendonsA-shi Acupoints plus stretching therapy to treat Scapulohumeral Periarthritisis significantly superior to needling A-shi points group and stretching group.3.Needling through the muscle tendons A-shi acupoints group to treat Scapul-ohumeral Periarthritis has no difference from stretching group. |
