To solve the problem of difficulty and expense of medical service, the resource distribution of basic public health service should be adjusted to be more equal and to cover whole city. According to the new medical reform of the state council that "cover the based, improve the primary, establish the mechanism", a new community health service model called family doctor service, one of the priority projects of the municipals government, has been developed since2011in Beijing. The family doctor service is provided by the community health service team. The service purpose is "popularize health knowledge, participate in the health action, provide health care, extend the healthy life" The team sign contracts with community residents to establish long-term service relationship, offer services and take responsibility for residents’health. The family doctor team offer active service which is more convenient and more efficient than traditionally passive service. The new service model can achieve higher equality and accessibility, which means great significance to basic health service for all. However, due to the deficiency of service mechanism, supporting measures and management, the development of family doctor service model is restricted.The current situation, problems and solutions of family doctor service in Chaoyang District is researched, with the purpose of providing reference to the development of family doctor service model, and of community and urban health service. The family doctor service model at home and aboard is described on the base of document research. The staffing situation and workload data from district health bureau is analyzed. The knowledge rate and satisfaction degree of sampled residents is analyzed by means of questionnaire. The problems of family doctor service are summarized through interview with staff working in community health service centers. Based on the analysis, problems of family doctor service and the solutions and suggestions are proposed. The research shows that every family doctor takes1348households in charge on average, which is far more than the quantity limited by government document. The average awareness rate of family doctor service is89.23%, while the contract rate is only42.55%.0.44health education materials is provided for each house per year, which is lower that the service standard-1material for each house per year. The health assessment is provided0.25times per capita per year, and health information notice is0.32times per capita per year, both fail the service standard-at least1time per capita per year. Health consultation and guidance service is provided0.93times per capita, which is lower than the4times standard. The satisfaction rate of contract resident is86.33%. The main deficiencies raise by residents include inadequate health assessment, inefficiency caused by doctor shortage, complexity of case registration and inconvenience of service. The service staff raise problems such as low intention toward the service of resident, inefficiency caused by doctor shortage, lack of facility and finance, low salary and stimulation and low level of information technology. Through research, five problems are raised:the lack of resident participation, insufficient allocation of medical staff, shortage of financial expenditure, deficient incentive mechanism and information mechanism. Solutions to the current problems of family doctor service, including the whole society participation, staff and finance support, incentive mechanism and information improvement, and have been raised.
