Objectives 1. To establish the models of the isogene rat's heterotopic pancreatic transplantation alone (PTA) with the ischemia/reperfusion (I/R) injury. 2. To investigate the effects of the I/R injury with the different period of the worm ischemia on the function of the donor's endocrine. 3. To analyze the influence of the I/R injury with different worm ischemia time on the pancreas cell apoptosis.Method 150 SD rats were used as donors and recipients with streptozotocin (STZ)-induced hyperglycemia, perform end-to-side anastomosis between the donors' abdominal aorta and recipients'abdominal aorta segment; and use a cuff to perform end-to-end anastomosis between the donors' portal vein segment and recipients' left renel vein. Pancreas exocrine drainages were reconstructed by the means of enteric drainage (E-D), the spleen vein was blocked so as to control the different worm ischemia time, to establish the models of the rat's PTA with I/R. The rats were sacrificed separatelly on 0.5h> 3h, 6h> 24h and 72h after reperfusion, simultaneously the blood and the donor' pancreas tissue were taked, the level of sugar in serum and donor' pancreas tissue concentration of Malondialdehyde (MDA),Reaction oxygen species (ROS)and Nitric Oxic (NO) were measured; histological changes of the donor' pancreas were studied; and the TUNEL cells were observed in different time after operations.Results 1. The blood glucose started to low at 3h after reperfusion; The blood glucose has not predominant change at 24h and 72h; the level of The blood glucose at 3h and 6h in the group of WIR 30 was higher than the group of WIR 0(P<0.01) and WIR 15(P<0.05) and the group of WIR 15 was higher than the group of WIR 0(P<0.01). 2. The level of donor tissue's ROS,MDA and NO started to change at 0.5h after reperfusion in each group, with the peak value at 3-6h when the level of ROS and MDA in group of WIR 30 was higher than thegroup of WIR 0(P<0.01) and WIR 15(P<0.05); the level of NO in group of WIR 30 was lower than the group of WIR 0(P<0.01) and WIR 15(P<0.05); and the group of WIR 15 was lower than the group of WIR 0(P<0.01). 3. At 0.5h after reperfusion the TUNEL cell was found in islets and the tissue around micro-vessels; and with the peak value at 6h when the apoptosis index(AI) of the group of WIR 30 was higher than the group of WIR 0(P<0.01) and WIR 15(P<0.05); and the group of WIR 15 was higher than the group of WIR 0 (P0.05) .Conclusions l.The longer worm ischemia means the more injury on the graft endocrine function; and 30min period of worm ischemia has had more injury on the graft endocrine function. 2. The toleration to I/R injury of the pancreatic exocrine is higher than the pancreatic endocrine. 3. The tissue of donors is injuried more severely with 30min period of worm ischemia when the cell injuris were represented the apoptosis.
