Objective: To investigate the early expression and significationof TNF-α and ICAM-1 in partial pancreas transplantation of rabbit and to study the protective effects of ulinastatin to ischemia-reperfusion injury after pancreas transplantation. Methed: The model of orthotopic partial pancreas transplantation of rabbit was used. They were randomly assigned to 3 groups:sham operation group, pancreas transplantation group, pancreas transplantation + ulinastatin group. The blood amylase and TNF-α in serum, MPO in pancreatic tissue were detected after 3 hour and 6 hour. RT-PCR was used to examine the expression of ICAM-1 mRNA in pancreas. The change of histology in graft was detected by H-E stain. Result:The level of blood amylase, TNF-α ,MPO in pancreas transplantation group were higer than sham operation group (p<0.05), the expression of ICAM-1 mRNA and the change of histology in this group were higer than sham operation group(p<0.05). The level of blood amylase, TNF-α ,MPO in pancreas transplantation + ulinastatin group were lower than pancreas transplantation group(p<0.05), and the expression of ICAM-1 mRNA and the change of histology in this group were lower than pancreas transplantation group(p<0.05). Conclusion: The increase of TNF-α can enhance the expression of ICAM-1. TNF-α and ICAM-1 can accelerate the aggregation of neutrophils so they all play a important role in ischemia-reperfusion injury after pancreas transplantation. Ulinastatin can slow done the expression of TNF-aand ICAM-1 so it have the protective effects to graft.
