| Objective:To primarily confirm the association between HLA-DRB1*07 and hepatitis B virus infection in a Guangdong population of the Han nationality by studying the association of HLA-DRB107 separately with the feature of response to hepatitis B vaccine,with the prognosis of spontaneous hepatitis B virus infection and with the feature of response to interferon-a (IFN-a) hi chronic hepatitis B individuals,and provide immuno-genetic knowledge for the prevention and cure of hepatitis B virus infection.Methods:Following people were recruted hi this study:(1) vaccine group:118 non- or hyporesponders,100 medium responders and 45 strong responders from 1400 healthy students who were subjected to a standard course of immunization with recombinant hepatitis B virus vaccine;(2) chronic HBV infection group:100 chronic patients,21 HBV spontaneously cleared individuals,and 70 healthy volunteers not exposed to HBV;(3) IFN group:69 chronic hepatitis B individuals from Guangdong of the Han nationality were subjected to a standard course of treatment with IFN-a. Peripheral blood IFN-y,EL-4 secreted by CD4+ T cells were detected by ELISA. HLA-DRB107 genes were detected with polymerse chain reaction-sequence specific primes(PCR-SSP). The data were analysed by SPSS 8.0 software.Results:(1) In the vaccine group,the frequency of non- orhyporesponse was 8.428% (118/1400),and that of hyperresponse was 3.214% (45/1400). The frequency of HLA-DRB107 gene in non- or hyporesponsers (15.25%) was higher than that in medium or hyperresponsers (3.45%) (P<0.05,OR=5.040);. (2) In the chronic HBV infection group,the frequency of HLA-DRB107 gene expression in chronic carrier group (CC) was 30%,which was respectively higher than that (4.76%) in spontaneously cleared group (SC) (P=0.016,OR=8.571) and that (8.57%) in unexposed group (UE) (P=0.001,OR=4.571);in chronic carrier group,the average IL-4 level in HLA-DRB107 positives was higher than that in HLA-DRB107 negtives (P<0.05),and the difference of IFN-y level between HLA-DRB107 positives and HLA-DRB107 negtives was not significant (P >0.05);(3) In the IFN group,the frequency of HLA-DRB107 was 17.39% (12/69) which was significantly higher than that hi the Guangdong healthy people. The frequency of HLA-DRB107 hi nonresponders was higher than that in the partial- or complete-responders(P<0.05,OR=18.55). After 6 months treatment with IFN-a,the IFN-y level was higher than that of pre-treatment for every patientnt while the IL-4 level decreased after treatment. The average changing extent of IFN-y level before and after treatment in the HLA-DRB107 negtives was more evidence than that in the HLA-DRB107 positives(P<0.01),and the corresponding comparison of IL-4 is also significant (P<0.01).Conclusions:In a Guangdong population of the Han nationality:(1) non- or hyporesponse to HBV vaccine is associated with the HLA-DRB107 gene;(2) HLA-DRB 107 is associated with hepatitis B virus infection-,(3) HLA-DRB107 is associated with nonresponse to IFN in chronic hepatitis B individuals,hi a word,HLA-DRB107 is associated with hepatitis B virus infection hi a Guangdong population of the Han nationality. |
