| Many surgery patients have psychological problem, for example; depression, anxiety. The psychological problems effect postoperative recovery. There are very little reports that applied psychological therapy in patients undergoing hysterectomy under epidural anesthesia. The study was designed to evaluate the effects of psychological suggestion therapy with Midazolam on recovery profiles by Symptom-Check-List-90 (SCL-90) in patients undergoing hysterectomy under epidural anesthesia.Material and MethodsI : Clinical Material100 ASA I - H hysterectomy aged from 20 to 60 years old patients were enrolled in this study. Patients with psychogenia, cerebral trauma, combined with other disease were excluded from this study. The patients were divided into two groups ( A, B ).II : MethodsAll patients were premedicated with luminal 0. 1g, atropine 0. 5 mg, intramuscularly. In group A anesthesia was induced with 2% li-docaine 5 ml epidurally, and maintained with 2% lidocaine. In group B anesthesia was induced with 2% lidocaine 5 ml epidurally, maintained with 2% lidocaine and Midazolam, applied psychological suggestion therapy.III: Life Monitoring:Before induction, Blood pressure, heart rate, respiratory rate, pulse oxygen saturation were recorded. The SCL-90 was used to evaluate psychological status of the patients on the sixth postoperative day.ResultOn the days of postoperation, patients with Midazolam and psychological therapy required less rescue analgesic. In group B symptom of the obsessive-compulsive dimension, the depression dimension, the anxiety dimension, the diet and sleep dimension were better than that in group A.DiscussionFor years, the surgery patients'psychological problem were more and more emphasized. How to improve the patients'psychological status is wildly studied. But the result was not satisfactory.The psychological suggestion therapy was used in many medical field, But now, only several reports show that the psychological suggestion therapy was useful in postoperative patients. Appling the psychological suggestion therapy during the operation has not reported. The patients undergoing hysterectomy have serious psychological problems , For example: depression and anxiety for operation, worrying thesexual function. There were meaningful, significant difference in postoperative recovery of patients between the two groups: receiving therapeutic suggestion and controls during the operation. The patients with psychological suggestion therapy required less rescue analgesia, and the symptom of the obsessive-compulsive dimension, the depression dimension, the anxiety dimension, the diet and sleep dimension were better. The study demonstrate that psychological factors effect pain. Pain is an uncomfortable feeling that tells you something may be wrong in your body. The pain-psychological problem-pains circle formed. Appling psychological suggestion therapy improve the psychological state, decrease the anxiety, depression, you will feel comfortable. At the same time, pain decrease too.The SCL-90 is a 90-item self-report system inventory developed in the 1980s by Derogatis and designed to reflect the psychological symptom patterns of community, medical respondents. In the special application of trauma and postoperation, the SCL-90 is particularly useful. It can validate or challenge the veracity of the patients claims, it can be used to follow the patients progress, and it can also be used as an variable outcome in clinical research.
