Association Pulsepressure With Microalbuminuria In Patients With Essential Hypertension

Objective: To investigate the relationship between pulse pressure (PP) and urinary albumin excretion(UAE) and microalbuminuria (Malb) in patients with essential hypertension. Method: This is a cross sectional analysis for a population sample of 58 patients with essential hypertension, including 27 male and 31 female. mean age of 63.19±3.62 years. Data collection included: Blood pressure,heart rate,body mass index,24 hours urinary albumin excretion ,fasting plasma glucose,plasma cholesterol,triglyceride,creatinine and plasma albumin. Pulse pressure was calculated as systolic blood pressure minus diastolic blood pressure. Microalbuminuria was defined as urinary albumin excretion of 30~300 mg/24h . The subjects were divided into three groups on the basis of the PP≤55 mmHg,56~75 mmHg and PP >75 mmHg. The 24h UAE and microalbuminura in three groups were compared respectively. Result: Findings were not significant different in sex, age, heart rate,body mass index, plasma cholesterol, triglyceride, creatinine andplasma glucose in three groups. The values of UAE in three groups were 12.54±9.18mg/24h, 23.09±11.5mg/24h, 29.97±5.25mg/24h respectively (P<0.001). Prevalence of microalbuminuria in three groups was 13.6%,25%,50% respectively (P<0.01),The increasing of UAE and microalbuminuria were accordingly with increasing of PP. UAE was postively correlated with PP( r =0.671 ,P<0.001) ,SBP( r =0.574 ,P<0.001) and weakly negatively related with DBP (r=-0.243, P<0.001). Pulse pressure of microalbumnuria group was 73.7±16.6mmHg significantly higher than that of nonmicroalbuminuria group(58.6±17.7mmHg).Conclusion: The level of UAE and prevalence of Malb were keeping with ascending level of pulse pressure in three groups,PP was greater correlation with UAE than SBP. The PP was a predictor of early renal damage in patients with essential hypertension.
