| Essential hypertension ( EHT) is a common cardiovascalar disease. EHT is a multiple - cause disease and one of the main diseases that do harm to peoples health and can lead to severe pathological changes of the patients'cerebro - vessels, the heart, the kidney and so on. So EHT has drawed increasing attention of researchers in recent years.Sullivan put forward the hypothesis that body iron stores are related to the risk of cardiovascalar disease in 1981. Several studies have been conducted since then to assess the hypothesis, especially the association between iron and atherosclerosis, CHD( coronary heart disease) or AMI (acute miocardial infarction). But the result was not consistent.A research by Piperno A et al. found that Hypertensive men with increased serum ferritin had more frequent than those with normal serum ferritin. But the number of the published studies on the association between iron and EHT was small and lacked of the reports on epidemiology research. Our research discussed the relationship of serum iron (SI) and EHT based on a large number of EHT patients and the control in order to put forward pathogenic research evidence on EHT.Subjects and Methods1. The selection method and standard of study population Using the method of clustering multistage sampling method, 6 countrysides were randomly selected in ZhanWu County, LiaoNing Province, and then randomly selected 2 villages in 5 countrysides and 1 village in 1 countryside. Allthe people above 18 years old in the selected 11 villages were investigated. Other conditions being able to induce serum iron increase or decrease unrelated to body iron stores such as acute hepatitis or malignancy etc. were excluded. And secondary hypertension was excluded also.2. The contents of epidemiological survey(1) Questionnaires were filled by means of inquiring and measurement in sites. The contents of survey included: general characteristics, alcohol intake, smoking habits, family history of hypertension, previous disease history and so on, measurement of the blood pressure, body height and weight.(2) Five milliliters blood samples were drawn from an antecubital vein after an overnight fast. After centrifugation, the serum fraction was removed and frozen in aliquots at - 70 until assayed. The serum was used for the measure of Serum Iron (SI) and other hemal-biochemistry assay related to EHT.3. Diagnosis standard and chemical measurementsThe diagnosis standard of EHT: According to 1999 WHO - ISH guidelines for the management of hypertension, hypertension was defined as a systolic blood pressure ( SBP) 140mmHg And/or a diastolic blood pressure ( DBP) 90mmHg. The measurement of blood pressure should be carried out according to the unified standard under standard conditions.SI, cholesterol ( CHOL ) , triglyceride ( TG ) , high density lipoprotein (HDL) , low density lipoprotein( LDL) , serum sodium, serum potassium, serum calcium were measured by automatic biochemistry analyzer 7150 ( HITACHI , Japan) , the blood sugar was measured by blood sugar analyzer( Johnson & Johnson, America).4. Selection of case and control groups(1) EHT Model; All 2021 EHT patients constituted the case group from the selected population, and the other 2351 healthy peoples formed control group.(2) High SBP Model: All 1569 high SBP(SBP 140mmHg) peoples constituted the case group from the selected population, and the other 2803 normal SBP peoples formed control group.(3) High DBP Model: All 1594 high DBP(DBP 90mmHg) peoples con-stituted the case group from the selected population, and the other 2778 normal DBP peoples formed control group.5. Statistical methodsA database was created by FoxPro after quality examination of data. Statistical analysis of dichotomous variables was by the chi - square test. Students t -test was used to assess differences between groups for two continuous variables. Kruskal - wallis test was used to assess differences between groups for multiple continuous variables allowingfor unequal va... |
