Objective To investigate the varity and mechnisim of the plasminogen activator inhibitor-1 activity in type 2 diabetes with hypertriglyceridemia.Methods The 4G/5G allele polymorphism in the PAI-1 gene promoter region were genetyped by allele specific PCR in 108 type 2 diabetes with or without hypertriglyceridemia and 40 normal controls.The activity of plasma PAI-1 was assayed by a chromogenic substrate method.In the meantime fast blood glucose,fast insulin,blood lipids,blood pressure,body mass indexs(BMI), homeostatic indexs of insulin resistance HOMA IR were studied.Results (1)The plasma PAI-1 activity in patients with hypertriglyceridemia was higher than that in the subjects without hypertriglyceridemia and controls (0.85 0.12vs0.64 0.09;0.9 0.11vs0.79 0.11 P<0.001).The level of increased PAI-1 activity was positively correlated with fast insulin, HOMA, BMI. (r=0.275, 0.293, P<0.01;r=0.203 P <0.05).(2)The frequency of PAI-1 4G/4G genotype was higher(55.6%vs38.9%),5G/5G was lower (16.7%vs33.3%) in type 2 diabetes with hypertriglyceridemia compared with diabetes without hypertriglyceridemia, but no statistic significance was present in two groups ( P > 0.05 ) .The frequency of 4G allele was higher in patients with hypertriglyceridemia than that in the other patients(69.4%vs52.7% P<0.05).(3)Plasma PAI-1 activity levels were significantly associated with 4G/5G polymorphism with the highest levels occurring in subjects with 4G/4G genotype, the intermediate levels in 4G/5G subjects, and the lowest levels in 5G/5G subjects(P<0.01).(4)The levels of PAI-1 activity regulated by triglyceride dependent on PAI-1 4G/5G genotype. The levels of triglyceride were significantly associated with PAI-1 activity only in 4G/4G group. The plasma PAI-1 activity was regulated by insulin in each group.Conclusion Metabolic determinants and 4G/5G allele polymorphism of PAI-1 were associated with the levels of the plasma PAI-1 activity; Hypertriglyceridemia can enhance this varity.
