Objective To study the smoking status and the cognize of smoking hazard of the middle school students in Hefei urban, to provide basis data and the golden rule for intervention measures. And to evaluate the effect of the intervention program performed in schools.Methods 6 292 students from 13 schools in Hefei urban were drawn to the study using cross sectional study method, who were asked to answer their smoking status, knowledge about smoking hazard, attitude towards smoking. The related risk factors were analyzed using chi-square test and logistic regression model. And 790 students had received a smoking prevention education program using a community trial.Results The total rate of smoking was 13.8%. Significant difference of the smoking rates was found between the male (21.6%) and female (5.2%), senior (15.6%) and junior (10.3%) students (p<0.001). The main factor that caused students to smoke is curiosity (44.8 %). The wrong attitude was the major reason for them keeping smoking. Most adolescent smoked with others, but few were heaven smokers. Knowing smoking harms health was the main factor that encourage them to abandon smoking. Students' cognize to smoking hazard and the attitude towards smoking were not good though most understood smoking is harm to health. The results of logistic regression analysis showed that boy, having smokers among surrounding population, misunderstanding on the harm of smoking, wrong attitudes towards smoking behaviors, higher grade, etc. might stimulate students to smoke. On the contrary, the fowling factors such as female, correct understanding on smokinghazard and correct attitudes towards smoking behaviors, showed positive roles to prevent smoking hazard. The results of univariate logistic regression analysis showed a general equal impact of the factors on both experimental smoking and current smoking, and the results of multivariate logistic regression analysis showed a significant difference of factors that affected the two smoking types. But a consistent impact of the factors was found, and the force of them was stronger when acting on current smoking. After intervention, the correct cognition to cigarette smoking of students had increased and the attitude towards smoking behaviors improved among the experimental group. The experimental smoking rate of intervention group showed a minor increase than that of control groups. Significant difference of experimental smoking rate was found between different groups. But, by comparison to pre-control group, the current smoking rate of experimental group had degreased contrast to inner-control group.Conclusions The term of high school is a key period for students to start smoking and convert to a related stable smokers. So it is important to carry out effxective prevention programs at high school period in order to decrease smoking rate and smoking harm.
