| Premature rupture of fetal membranes (PROM), one of the common complication in pregnancy, means that the fetal membranes have already ruptured before stages of labor, the morbidity of it account for 3% to 16% of the total childbirth. It is one of the reason that leads to premature delivery, chorioamnionitis neonatal infection and neonatal asphyxia. For discussing the relationship between the PROM and the complications in mother and baby, we analysed in review the cause of PROM, manner of parturition, pollution of amniotic fluid, apgar score and treatment in 50 cases to be pregnant 28-40 weeks from January, 2000 to Octobe, 2002. We also selected in random 40 nomal parturien as contral team in same hospitalization period. The other complications was eliminated in each of teams. The result display that the cause of PROM are, the many times cyesis in 62%, cephalopelvic disproportion in 46%, abnomal fetal position in 10%. Sexual intercourse in 6%, vaginitis in 6%, second PROM in 4%, hydramnios in 2%. The rate of cesarean section in PROM (33cases) is 66%, more than one of the contral cases that is 40%(20cases). The pollution of the amniotic fluid in 8 cases (16%) went beyond 2 degree, more than mainly contral team in 2 cases (4%). Apgar score in PROM was lower than contral team. P<0.05, having signi ficant difference. The fetal position was different in two teams. In PROM, occiput posterior position was 18 cases (36%). occiput horizontal position was 10 cases (20%). Breech position was 3 cases (6%). In contral team,. occiput posterior position was 9 cases (18%). Occiput horizontal position was 5 cases (10%). Breech position was 1 case (2.0%) comparison of above all in the two teams, P value<0.05, having significant difference. When the time of membrane rupture exceed more than 24 hours, 5 cases (55.55%) premature infant, 5 cases (55.55%) neonatal asphyxia, 3 cases (33.33%) neonatal infection were reported in PROM. When the time of membranes rupture was less than 24 hours, we found that the 4 cases (9.76%) preterm infant, 3 cases (7.32%) neonatal asphyxia, 1 case (2.44%) neonatal infection P<0.05. having significant difference. Above all, PROM increase the opportunity of cesarean section, premature birth, neonatal asphyxia and neonatal infection. If treatment was correct such as enhance and reinforce monitor of the antepartum extend the pregnant period whose less than 37 weeks used reasonably the antibiotic and steroid hormones and end the pregnancy in time. Meanwhile, complication in mother and baby would be decreased by ehhancing and reinforcing the care for the premature and the neonate and preventing the diseases. |
