Operative Treatment Of Acetabular Fractures

Objective Acetabulum has deep positions and complex neighbourhood relations. Acetabular fractures are injuries with high speed and high capacity.Therefore the situations of acetabular fractures and associated injuries were complicated.Closure reduction and traction therapy were adopted to treat the acetabular fractures before.But it is difficult to arrive at the aims of excellent reduction and it is apt to traumatic arthritis.In recent years the displaced acetabular fractures were treated with operation therapy.This article is to discuss the diagnosis and operation and how to prevent the complications of acetabular fractures.Methods 26 cases of displaced acetabular fractures were performed X-ray sheet, CT and 3D-CT before operation in order to determine the classification of fractures and how to operate. All the cases were fixed by reconstruction plates through the best approach.Results 2 cases with injuries of skull and brain who were confused state of mind in 26 cases were missed diagnosis in emergency room because pelves X-ray sheets were not accepted. 26 cases were followed up for 1-4 years, with an average of 2 years.According to Mttas criteria clinically, 14(55%) excellent,8(30%) good,4(15%) fairs.The rate of excellence and goodness was85%.The major complications were sciatic never injury, traumatic arthritis, and heterotopic ossifacation.Conclusions The principle of the treatment in displaced acetabular fracture is precise reduction, firmly internal fixation, early functional training. The key to enhance the effect and improve the reductive quality of acetabular fracture is to verdict the type of fracture and select the best approach .
