Immunophenotype Characteristics And Expression Of Antiapoptotic Gene Bcl-2 In Hodgkin Lymphoma

Reviewing the 50 HL cases with some results done by the others,using immunohistochemical method to mark CD20,CD45RO,Bcl-2 for all the cases and CD79a,CD3,CD68,EMA,LCA for some cases,we found that except one patient suffered from anaplastic large cell lymphoma and the other one can't be defined because of lack of adequate tissue and tumor cell.According to the classification of WHO(2001),the rest were defined classical Hodgkin lymphoma.We also defined the subtypes for all the 48 cases and studied the correlation between B,T immunophenotype of HRS cells,reactive background lymphocytes,expression of Bcl-2 protein and sex,age, histological subtypes and/or expression of EBV. It revealed that the ratio of male to female was 1.09:1 and the site was most seen in cervical lymph node (64.58%). We identified that the percentage of MCHL was the most in all the four subtypes,the percentage of NSHL equaled to that of LRCHL, LDHL was the least.Different immunophenotypes were expressed in HRS cells,B line reached 33.35%(1/3),T line added up to 54.17%(26/48).B immunophenotype is significantly associated with histology subtype (P﹤0.01),but no correlation is found between B immunotype and sex,age and expression of EBV (P﹥0.05).T lymphocyte predominant in reactive cells accounts for 87.5%,B lymphocyte being 12.5%.Reactive lymphocytes are greatly associated with sex and age (P﹤0.005),but not relevant to EBV expression (P﹥0.05).Bcl-2 gene expression was found in 31/48 cases(64.58%),whose percentage is more in NSHL than MCHL(84.62%;65%) and also more in T immunophenotype than in B immunotype (73.08%;50%).Bcl-2 gene expression is greatly relevant to EBV expression (P﹤0.005),not to sex,age and histology subtype (P﹥0.05).These results suggest that (1) different immunophenotypes are expressed in HRS cells,but B and T are most, T more than B in percentage;(2)T lymphocyte predominant in reactive cells acconts for 87.5%,B lymphocyte being 12.5%;(3)there are approximately 2/3 cases showed Bcl-2 protein positive;(4)significant correlation:①the expression of B immunophenotype in HRS cells and histological subtype.②the expression of Bcl-2 protein and that of EBV gene.③reactive lymphocytes and sex,age.In conclusion,we learned about the histological subtypes on young adult HL in this district and the immunophenotype characteristics of HRS cells. In order to avoid uncorrect diagnosis,except the histological and immunological characteristics of HRS cells should be grasped,the elements of reactive lymphocytes also be emphasized.Correct diagnosis can't only depend on the results of IHC.In the meantime,combing with the expression of Bcl-2,we also provide predictive theory for the treatment and prognosis of HL patients in this district.
