| Object To investigate the optimal concentration and the proper dosage of each parent component when the local anaesthetic mixture of chloroprocaine and ropivacaine was used in epidural anaesthesia .Methods Sixty patients (ASA I - II) undergoing gynaecological operation were randomly divided into four groups which are labeled group R, group C1-R1 , group C2-R1 and group Ci-R. In group R (n=15), The agent of 0.75% ropivacaine was used in epidural blockade,And the mixture of 1% chloroprocaine and 0.5% ropivacaine was used in group C1-R1(n=15); The mixture of 1.5% chloroprocaine and 0.5% ropivacaine was used in group C2-R1(n=15); The mixture of 1% chloroprocaine and 0.75% ropivacaine was used in groupC1-R(n=15).Observing and notingrthe onset , duration time and the highest level of sensory blockade;the degree of motor blockade using Bromage scale, the onset and duration time of motor blockade,and the others including assistantal medication in operation, postoperative complication and satisfying rate of patients.Results As compared with baseline, mean arteral blood pressure(MAP) in each group decreased at 20 min- 30 min, 40 min and 50 min after the agents were injected into the epidural space, p<0.0l.No severe hypotension case occurred. As compared with group R,the onset of sensory blockade achieved to the T12level, T8 level and the highest level was significantly shorter in group C1-Ri,group C12-R1 and group C1-R,P<0.01.Each patient got motor blockade of Bromage3 ingroup R and group C\-R.; In group Ci-Ri,12 patients got motor blockade of Bromage3,the others: 2 patients got motor blockade of Bromage2, one got motor blockade of Bromagel only.And in group C2-Ri,14 patients got motor blockade of BromageS , another one got motor blockade of Bromag2 only. As compared with group R,the onset of motor blockade of BromageK Bromage2 was significantly shorter for group Ci-Ri group Ci-Ri and group Ci-R,P<0.01 .The onset of motor blockade of Bromage3 had no significantly different among group R group Ci-Ri and group C2-Ri, P>0.05. And it was significantly shorter for group Q-R as compared with group R, group Ci-Ri and group C2-R, p<0.01. As compared with group R ,the duration of sensory blockade and motor blockade was significantly shorter for group Ci-Ri group Ci-Ri, p0.0l. There was no significantly difference for the rate of using ephedrine and atropine among the four groups, P>0.05.But the dosage of using ephedrine,group Ci-R was increased as compared with that in the other groups. Either rate or dosage of using ketamine.,group Cj-R was decreased as compared with every other groups,p<0.01. There was no significantly difference for the rate of back pain postoperatively and the satisfying rate of patients among the four groups.,P>0.05.Conclusion Onset of sensory blockade and motor blockade of Bromage2 was faster with the mixture of 1.5% chloroprocaine and 0.5% ropivacaine and the mixture of 1% chloroprocaine and 0.75% ropivacaine used for epidural blockade.The mixture of 1.5% chloroprocaine and 0.5% ropivacaine and the mixture of 1% chloroprocaine and 0.75% ropivacaine are suitable for epidural blockade performed to gynaecological operation better. |
