Purpose:To explore the correlation between HPV and metastasis of lymphonode. To evaluate the significance of Cytokeratin19(CK19) expression in diagnosing micrometastases of lymph nodes in patients with laryngeal squamous cell carcinoma.Methods:The morphology for 148 cases of laryngeal squamous cell carcinoma and lymphonode were analyzed under the microscope. Forty cases of laryngeal carcinoma together with 163 lymph nodes were studied by the way of staining with hematoxylin and eosin(HE)and immunostaining with antibody against cytokeratin 19(CK19).Results:1 The expression of koilocytoid cell in laryngeal carcinoma and the lymphonode(l)The positive rates of koilocytoid cell in laryngeal carcinoma and lymphonodes are 73.6%(109/148) and 47.3% (70/148) rsepectively.(2) There is significant difference between wether or not metastasis lies in lymphonode,no significant difference is found in sex.(3) There is significant difference between laryngeal carcinoma and metastatic lymphonode .2 The expression of CK19 in laryngeal carcinoma and the lymphonode(1) The positive rates of CK19 in laryngeal carcinoma and lymphonodes are 100%(40/40) and 25%( 10/40) rsepectivly.(2) Micrometastases of lymph nodes show a positive correlation withpathological stage. There is significant difference between T1,T2 and T3, T4.There is significant difference in CK19 expression between wether or not metastasis of lymphonode, (3) There is significant difference between HE staining and CK19 detection.Conclusions:(l)HPV plays an important role in the metastasis of lymphonode for laryngeal carcinoma.(2) The expression of koilocytoid cell without pathological neck lymphonode metastasis shows the possibility of cervical lymph node metastasis.(3) Immunohistochemical staining on CK19 is a valuable method for detection of node micrometases in patients with laryngeal carcinoma.
