Background: Splenic tumor is a rare disease and is scarcely mentioned in the literature. Here, we studied on 50 patients treated in the First Affiliated Hospital of Zhejiang University and wish to analyze the natural history of these disease , to investigate the measures of diagnosis and treatment and to summarize the experience in diagnosis and treatment of splenic tumors,Methods: 50 cases , including 26 cases of benign tumor and 24 cases of malignant tumor, admitted into our hospital from January 1991 to September 2004 were followed up, and clinical features, managements and follow-up results of these patients were analyzed retrospectively. All statistical analysis were performed with SPSS 10.0 statistical package. Survival figures were estimated by the Kaplan-Meier method and differences groups were tested by the log-rank test. The Cox multivariate analysis was performed to determine prognostic factors for improved survival.Results: Among 50 patients, 47 were treated by splenectomy without any operative mortality and postoperative severe complications, 3 benign tumor patients were treated by partial splenectomy, 15 malignant tumor patients received lymph resection at the same time, and 13 malignant tumor patients received chemical therapy after operation. The overall 5-year survival rate of thebenign tumor was 100%, while the l 3 , and 5-year survival rate following radical resection for malignant tumor was 66.68% 46.75% 24.24%, respectively. Splenic metastasis tumor has poor prognosis, Multivariate analysis revealed that histopathological type and therapy measure had obviously relation with splenic malignant tumor prognosis (P =0, P =0.04)Conclusions: It is difficult to distinguish the character of tumor before operation; Splenic malignant tumor prognosis is not good, lymph resection may help to raise the survival rate; Radiotherapy or newer chemotherapeutic agents may improve the prognosis; Splenic metastasis tumor has very poor prognosis,, ephasis to improve the long-term survival should be paid on original tumor management.
