The incidence rate of Chronic Urticaria is high in thedermatology department ,it is difficult for doctor to treat.Inrecent years ,the study about the Chronic Urticaria was madegreat progress . Some studies indicated that the function of Thelper cell subpopulation is unbalanced ,which lead to allergedisease .Its mechanism is mostly a immunity course by IgE andthe immunity reaction relates to the release of gene andmedium ,In the past ,the researchs make the keystone on thescale change of IgE/IgG and domine offect cell amount .Atpresent , many searchers judge the amount of the Th1/Th2indirectly by detecting the content of IFN-γor IL-4, etc , both inbasic search and clinical search . In 1995, Del found thatCD30 superfic signal on the surface of Th2 cell and themasculine rate of CD30 in Th2 cell was higher significantlythan that in Th1 cell ,So it was possible to detect Th1and Th2directly through its specific signal on surface.In this research,wecollect 60 patient with chronic urticaria and made one groupdivides into two groups,one group being given Mizolastine onlyand other group being given Mizolastinegive and Tripterygiumwilfordii.The aim is to research the level of Th1/Th2 and IgE inChronic Urticaria .Objective:To research the level of Th1/Th2 and IgE inChronic Urticaria .Materials and methods: 60 patients with chronic urticariawere collected from the dematology department of the FourthAffiliated Hospital of Hebei Medical University from 2003.11 to2004.8. All patients were classic Chronic Urticaria and beingselected,excludingSLE,pemphigus dermtomyositis ,autoimmunediseases and other allergy disease.Antihistaminic drugs was nottaken by patient group for more than three days.20 healthypersons were taken as normal controls .At same time,we collect5ml blood from patients .Th1 and Th2 cell number and IgE levelbetween Chronic Urticaria patients befor treatment and aftertreatment as detected by flow cemetry and ELISA.Results : 1. After treatment, most patients symptomremitted the SSRI (symptom scor reduce index)decreasedsignificantly.The combine group effective rate was 90%, and thesingleness group effective rate was 70% ,no side effect wasfound.2. The express rate of CD4+CD30+(Th2) in combine groupchronic urticaria patients(22.98 ±1.43 %) was significantlyhigher than that in the normal people(9.25 ±1.02%)( t=2.463,P<0.05) have evidence difference; and the patientbeing treatment(10 ±1.32%) and comper with patients(t=3.164P<0.01)also have evidence difference;The singlenessgroup chronic urticaria patients(23.28 ±1.23%) wassignificantly higher than that in the normal people (9.25±1.02%)( t=2.591,P<0.05) have evidence difference; whereas thepatient being treatment(19.13 ±1.13%) and comper withpatients(23.28±1.23%)(t=1.783,P>0.05) no evidence difference;(Table1 and table 2)3. The express rate of CD4+CD30-(Th1) in combine groupchronic urticaria patients(75.92±1.21%)was significantly lowerthan that in the normal people(90.75±1.83%) ( t=3.012,P<0.01)have evidence difference.and the patient being treatment (89.85±0.72%) and comper with patients (75.92 ±1.21%)(t=3.575,P<0.01) also have evidence difference; The singlenessgroup chronic urticaria patients (76.72 ±1.02%) wassignificantly lower than that in the normal people (90.75±1.83%)( t=2.931,P<0.01) have evidence difference;whereas thepatient being retreatment(79.58 ±0.32%) and comper withpatients(76.72±1.02%)(t=1.321,P>0.05)no evidence difference.(Table 3 and table 4)4. The serum level of IgE in combine group chronicurticaria patients (302 ±125 (IU/ml)) was significantlyincreased compared to normal controls (95 ±170(IU/ml))(t=3.66,P <0.01).after treatment ,the level of IgE in chronic (110±95(IU/ml)) have evidence difference( t=2.98,P <0.01);Theserum level of IgE in singleness group chronic urticaria patients(299±108(IU/ml)) was significantly increased compared tonormal controls ( 95±170(IU/ml) ) (t=3.58,P<0.01).Aftertreatment,the level of IgE in chronic (120±110 (IU/ml)) haveevidence difference(t=2.42,P <0.05).( Table 5 and table 6)...
