[Abstract] Objective To compare the effect of remifentanil and fentanil in controlled hypotension for endoscopic sinus surgery(ESS). Methods Forty patients undergoing ESS were randomly divided into study group and contrast group with 20 cases in each group. Study group (n=20) used remifentanil for combined anesthesia and contrast group (n=20) used fentanil for combined anesthesia. Nitroglycery compound with esmolol was used for controlled hypotension in both groups to control mean arterial blood pressure(MAP) to 70% of the base data. But mean arterial blood pressure was maintained with no lower than 55 mmHg in both groups. To record all data of HR,MAP,RPP at every time point ,and operation time, blood loss,dose of nitroglycery and esmolol,time to extubation. ResultCombined anesthesia with remifetanil or fentanil can finish the target of controlled hypotension for ESS. But remifentanil group can reduce the doses of nitroglycery and esmolol, can more quickly adjust the depth of anesthesia at the right moment with stimulus intensity of operation. Remifentanil anesthesia can have more swif palinesthesia after operation,and have better reliability for anesthesia and operation. Conclusion Remifentanil is more suitable than fentanil in controlled hypotension for Endoscopic sinus surgery .
