| The symptom of depression in climacteric women is an representation whichonset before or after menopause,namely depression or sadness, at the sametime with thinking and action obstruction, mainly behave asgrief,weeping,instable mood.,indifferent or worried to what happened aroudher and even insomnia.Along with the prolonging expectation of women'slife-span,women will live their last 1/3 lives in menopausalperiod.,therefore the physical and psychological problems whichsolicitation because of menopause are turning into the hotspot of theresearch of women's health care .In the aspect of psychology,the arises ofsymptom of depression is constant and the incidence is quite high ,the reportsof the incidence of climacteric depression in aboard is up to 43%[1],andat home is 32%[2],The arises of this kind of depression severely influencetheir health of body and mind,which bring hard burden to their families andthe society .In recent years, the etiology of the climacteric depression symptom inwomen and the preventable measures have already aroused the attention of thescholars at home and in broad,but the exact pathogeny is not very clear.Atpresent there are a few theories about the pathogeny of the climactericdepression symptom in women :1.The theory of estrogen withdrawl:this theoryhypothesize the low estrogen state can drive the beginning or deteriorationof frame of mood of those who have the danger of becoming depression inclimacterium[3],but the researches indicate that there are no significantdifferences between the perimenopausal women either with or withoutdepression,[4,5], but Schmidt's[6] research indicate that the estrogenreplacement therapy's available effect to the perimenopausal depressionhave no relation to the available effect of the estrogen to the vasomotorsymptoms,so the mechianism of estrogen improve the the depression symptomof the climacteric women still needs further study. 2.Domino Theory:thephysical symptoms such as hot flushes result in inability to sleep which inturn leads to the emotional changes during the daytime[3]3.The effect of theneurotransmitters:some researches indicate[7]that the estrogendistribution in pituitary hypothalamus is quite high,the level of estrogendecrease after menopause,naturaly can influence the nerve movement of thecortex of cerebra and hypothalamus,which can decrease the activity ofadrenalin ,dopamine ,and aptide which lead to the in abnormities in mood andaction.Some researches indicate that the depletion of 5-HT can evolve oraggravate the mood of depression or anxiety[8].4.The effect of physiologicaland societyfactors:At home ,two researches indicate that there is nocorrelation between the onset of the symptom of depdression in theclimacteric women and menopause,but are correlated with the occupation,theincomes of their family and unsatisfied with their job and livingconditions[2,9]. At present the researches of the etiology of the depression symptom inclimacteric women are focused on the depletion of estrogen and the societyand psychological factors,the study of the progesterone andneurotransmitters is only a few ,and the study of the nutritional elementsis fewer.In this research we will further explore some factors such as serumprogesterone,folate and 5-HT,which are related to the symptom ofdepression ,the conclusions followed as:1. The symptom of depression in theclimacteric women is related to the level of serum progesterone,manyresearches indicate that progesterone play a role in the pathogenesis ofdepression[11], Meir[12]believe that progesterones can depress theCNS(central nerve system),progesterone can elevate the concentration ofmonamine oxidase(MAO) activity and decrease the levels of 5-HT[13],andprogesterone can reverse estrogen induced receptor expression[12],thismaybe the reason of progesterone can result in negative mood.In thisresearch,the level of serum progesterone is significant lower than thereproductive normal womenn ,accord with the changes of the endocrine inclimacterium,the level of progesterone in the depression group is higherthan the normal group of the climacterium,the difference is significant,suggest that the arise of depression in climacterium is related to thelevel of serum progesterone,the level of serum progesterone of the depressiongroup is relatively higher,therefore the ratio of estrogen to progesteroneis malajustment, so the abnormity of the ratio of estrogen to progesteronemaybe one of the reseasons of the high incidence of depression during thisperiod.2 The symptom of depression in climacteric women is related to thereduced serum level of folate,because in this research the serum level offolate in the group of climacteric women with depression is lower than thenormal climactic women and the normal reproductive women,what's more inclimacteric women serum folate is positive correlated with the serum 5-HT,andfolate involve in the cycle of one carbon and provide methyl to theneurotransmitters of single amines ,and involve the composed of severalenzymes and neurotransmitters,supply folate can accelerate the methylreaction ,catalyze the composed of many coenzymes andneurotransmitters,increase the content of 5-HT,elevuate the level of5-HT,therefore can resist depression [22],in this research the lower levelof 5-HT in climacteric women with depression whether is correlated with thedecreased folate need further research.3.The depression symptom in theclimacteric women is correlated with the decreased serum 5-HT,the arises ofdepression symptom maybe related with the reduced serum 5-HT during thisperiod.As for the perimenopausal women ,the failure of the ovary functioncan result in the endocrinal changes,this kind of endocrinal changes can leadto the hanges of neurotransmitters including 5-HT[30].Although manyneurotransmitters involve in the appearance of depression,the serotoninsystem play a role in the onset of depression[31]. During climacterium ,theserotonin changes is one of the changes of the neurotransmitters,but it maybethe important reason of the the appearance of depression in climactericwomen.MAO,5-HT and the ovarian hormones are interact with each other , theovarian hormones regulate the MAO's transcriptional and either play a rolein the neurotransmitters including serotonin.Therefore ,in this researchthe relatively higher serum progesterone of the group of depression ,canincrease the concentration and the activity of MAO,can also result in theanalyzing and metabolizing dopamine.noradrenalin and 5-HT,which can lead todisfunction of the brim system of midbrain,reticulation structure,and themidline of the lower gyrus ,so the depression symptom appears.4.The serumprogesterone is negative correlated with the serum 5-HT,folate is positiverelated with the serum 5-HT during climacterium. |
